GISTs are rare mesenchymal tumors, which may present as millimetric lesions or bulky masses. They are usually clinically silent until they reach a significant size, so their finding is often incidental. We report the case of a 92 – year – old female patient admitted to the Emergency Department due to clinical symptoms of right hypochondrium (RH) pain, nausea and vomiting, developing within a few hours. Clinical examination revealed tenderness in the RH and a palpable painless mass in the epigastric/left upper quadrants, of 10 x 10 cm. Blood tests and ultrasonography confirmed the diagnostic hypothesis of choledocholithiasis and also revealed a solid, heterogeneous mass of 14 x 8 x 9 cm, with cystic areas and cleavage planes. Endoscopic retrograde cholangiopancreatography (ERCP) was performed with endoscopic sphincterotomy (EST) and removal of calculi with no complications. The patient had open surgery so as to approach the gallblader and the mass at the same surgical time. Gastric wedge resection and cholecystectomy were performed. The histological and immunohistochemical analysis of the lesion showed a high-risk GIST. Due to the advanced age of the patient and the existing co-morbidities, it was decided not to perform adjuvant therapy with imatinib, as usually indicated. Review of literature show that giant GISTs of the stomach are rare and usually symptomatic. Complete surgical resection is the standard treatment in locally resectable GISTs, with adjuvant therapy being indicated for high-risk tumors.

Serum levels and tumor expression of TIM-3 as a promising clinical biomarker in cats with mammary carcinoma

    Publication
    by Valente, Sofia Seixas Morais
T-cell Immunoglobulin and Mucin-domain containing molecule-3 (TIM-3) is an inhibitory receptor with a key role in regulating inflammation, autoimmunity and cancer progression. Recently, in order to overcome resistance to current immune checkpoint blockade therapies, TIM-3 has emerged as a promising targeting molecule for cancer immunotherapy. Although feline mammary carcinoma (FMC) is a valuable cancer model, no studies on the TIM-3 receptor have been developed in this species. Thus, in this study, 48 cats with mammary carcinoma were evaluated. Firstly, immunohistochemistry was performed to assess the TIM-3 expression in total (t), stromal (s) and intratumoral (i) tumor-infiltrating lymphocytes (TILs) and in cancer cells. In parallel, serum TIM-3 levels were quantified and compared with healthy controls using enzyme-linked immunosorbent assay (ELISA). Finally, the TIM-3 gene was sequenced from 19 tumor tissue samples to identify putative mutations. In FMC, our results demonstrated that sTILs-TIM3+ are more frequent than iTILs-TIM-3+, which are more abundant in luminal A/B subtypes. A positive correlation between sTILs-TIM-3+ and cancer cells-TIM-3+ and higher tumor malignancy grade was found (p=0.044 and p=0.018, respectively), whereas higher iTILs-TIM-3+ densities were mostly associated with a benign tumoral behavior. Additionally, higher densities of cancer cells overexpressing TIM-3 were associated with lymph node metastasis (p=0.025) and triple negative basal-like (p=0.038) and luminal B carcinomas (p=0.010), in contrast with luminal A mammary carcinomas. Finally, higher tTILs-TIM-3+ densities were predominantly associated with more favourable clinicopathological features, namely the percentage of tTILs-TIM-3+ was negatively associated with tumor metastasis (p=0.010) and tumor recurrence (p=0.013). Moreover, the TIM-3 serum levels were lower in the cats with mammary carcinoma than in the healthy group (p<0.001), being positively associated with higher densities of tTILs-TIM-3+ (p=0.016) and sTILs-TIM-3+ (p=0.023). From sequence analysis, only one mutation was identified in one tumor sample (intron 2: NC_058368.1: g.190379339 G>A). Altogether, in line with previous findings in human reports, our results suggest that the expression of TIM-3 among TILs subpopulations and cancer cells may differently influence the clinical outcome of cats with mammary carcinoma, reinforcing the hypothesis that feline mammary carcinoma is a valuable cancer model

Utilização de resíduos orgânicos na reciclagem dos nutrientes e regeneração da produtividade dos solos

    Publication
    by Valente, Sofia José Fenes

Identification and Manipulation of Cystatins to Control Mycobacterium tuberculosis Infection of Human Macrophages

    Publication
by Valente, Sofia Margarida Serafim Carvalho Xavier
Tuberculosis (TB) is a disease caused by the facultative intracellular species Mycobacterium tuberculosis (Mtb), one of humankind’s most ancient and successful pathogens. The eradication of Mtb infection has been a problem over the ages, due to pathogen’s ability to survive in host cells and escape the immune defense mechanisms. One efficient mechanism to kill pathogens occurs within macrophages, innate immune cells that use, among others, phagocytosis, a mechanisms by which the macrophage engulfs the bacteria in a structure called the phagosome which matures and in which a series of killing events will the clear pathogens. Specifically, the content of the phagosome will acquire proteolytic enzymes, as is the case of cathepsins, that will take relevant role in pathogen destruction at the low pH environment of the phagolysosome and, not less relevant, processing pathogen antigens to be presented to lymphocytes and therefore to expand the immune response. Recent studies revealed that Mtb is able to regulate cathepsins at the level of the gene expression and activity, increasing its survival inside macrophages. Cathepsins are regulated by their inhibitors, cystatins, which bind their active site and down-regulate cathepsins activity. Since Mtb was shown to down-regulate cathepsin activity we hypothesize that the cystatin pathway might be explored to improve the processes dependent on cathepsin activity such as pathogen destruction and antigen processing by type II HLA complexes, to counter the ability of Mtb to survive during infection. In the first part of the study we target cystatin C, since it is described to be a major inhibitor of cathepsin S, an important cathepsin enrolled in pathogen clearance and antigen presentation. By silencing the mRNA of cystatin C we do hypothesized to prevent it from inhibiting cathepsins activation during several models of mycobacteria infection including the vaccine species Mycobacterium bovis bacillus Calmette-Guérin (BCG) and the non-virulent species Mycobacterium smegmatis infection. Overall, our results after cystatin C silencing, indicate an increase in proinflammatory responses by infected macrophages, concomitant with an increase of mycobacteria killing, and an improvement of the expression of HLA-DR receptors on the cell surface. In parallel, we focused on the influence of protease inhibitors (PIs) used in HIV therapy, as an alternative model to cystatin silencing, to control cathepsin activity within macrophages during mycobacterial infections. We assessed the effects of saquinavir and ritonavir on the antigen presentation HLA class II complexes expression at the cell surface and on the mycobacterial killing/survival effects. We found notable interactions between the infected cells and the protease inhibitors, particularly saquinavir was observed to increase the expression of HLA-DR on the cell surface and increased mycobacterial killing, while ritonavir decreased HLA-DR expression on the surface and contributed to an increased mycobacterial survival. Altogether our results show that by manipulating cystatin C expression in macrophages and/or the treatment of infected cells with PIs, leads to macrophage interference of cathepsins and therefore with macrophages defense mechanisms. Thus suggests that cystatins and PIs manipulation are a potential tool to improve the host cellular immunity against Mtb and be used as an host-targeted therapeutic.
