Ophthalmology emergency room at the University of São Paulo General Hospital: a tertiary hospital providing primary and secondary level care
Publicationby Carvalho, Regina de SouzaOther Authors: José, Newton KaraOrigin: OasisbrOBJECTIVE: To assess the need for emergency care in a tertiary hospital, in the Ophthalmology Emergency Room of the University of the São Paulo Medical School General Hospital. MATERIALS AND METHODS: A cross-sectional analytic study of the treatment complexity level was carried out in a readily available sample, n = 574, of patients seen at the Ophthalmology Emergency Room of the University of São Paulo Medical School General Hospital, during a typical week. RESULTS: Of the 574 treated patients, 69.0% of the cases were diagnosed as requiring a simple treatment for their problem. The most frequent diagnoses were related to ocular inflammation and infection (55.0%), mainly including conjunctivitis (29.4%) and eyelid inflammations (10.5%). Next in frequency were ocular trauma (19.2%), particularly foreign bodies on the cornea (7.5%) and blunt trauma (5.2%). Refractive errors (3.1%) was the most prevalent diagnosis in the "other ocular modifications" category. CONCLUSION: Most of the cases (69.0%) treated at the Ophthalmology Emergency Room of the University of São Paulo Medical School General Hospital could have been diagnosed and treated in primary or secondary care units. Also, the fact that return visits to check recovery are seen in the ER demonstrates the lack of referral services. The fact that patients come to a tertiary hospital with rather simple cases shows the poor structure of the Brazilian Public Healthcare System, which overloads the tertiary care facilities, where costs for human resources, materials, and other items are higher. -

Patterns of detection and optical correction among ophthalmologists
Publicationby Carvalho, Regina de SouzaOther Authors: Kara-José, Newton; Temporini, Edméa Rita; Kara-Junior, NewtonOrigin: OasisbrOBJECTIVE: To verify age of first ophthalmic evaluation and optical prescription along with present optical correction among ophthalmologists of different age groups. METHODS: A cross-sectional study was conducted with a nonprobabilistic sample (n = 578) of ophthalmologists and resident physicians using a self-administered questionnaire. RESULTS: The first ophthalmologic exam was undergone before age 7 for 33.3% of the ophthalmologists aged between 23 and 30 years, for 25.8% of those aged from 31 to 42 , and for 15.8% of those aged from 43 to 76 years (P < 0.0005). The first ophthalmologic exam was undergone at 8 to 22 years of age for 60.7% of the ophthalmologists aged between 23 and 30, for 54.9% of those aged between 31 and 42, and for 47.4% of those aged from 43 to 76 years of age. Use of the first optical prescription (eyeglasses) between 0 and 7 years was reported by 10.0%, from 8 to 20 years of age by 59.6%, from 21 to 40 years by 18.1%, and 41 years or older by 12.3% . Present use of optical correction was reported by 69.0%. Concerning type of corrective device chosen, 63.7% wore only eyeglasses, 29.8% wore eyeglasses and contact lenses, and 6.5% wore contact lenses only. CONCLUSION: Among the sample of opthalmologists, the first ophthalmic evaluation and corresponding optical correction usually occurred relatively late (8 to 20 years of age). However, in the younger group of ophthalmologists, there was a highly significant increase in the number of subjects who had undergone an ophthalmologic exam before age 7. Eyeglasses were reported as the correction of choice by those in all age groups. -

Assessment of visual health campaign activities at schools: teachers' perception
Publicationby Carvalho,Regina de SouzaOther Authors: Temporini,Edméa Rita; Kara-José,NewtonOrigin: OasisbrPURPOSE: 1) To assess the opinion of teachers involved in visual screening of school-age children and their referral to ophthalmologic examination within the "Eye-to-Eye National Campaign for Visual Disorders Prevention and Rehabilitation" (Campanha Nacional de Prevenção e Reabilitação Visual Olho no Olho); 2) To identify the teachers' perception with respect to the training received for conducting the campaign. METHODS: A descriptive study has been conducted, based on data recorded from the application of a questionnaire to 1,517 elementary school teachers working in public schools in 27 Brazilian states. The following items were examined: received orientation, use of educational videos and handbooks, supplied explanations, difficulties, questions and overall campaign assessment. RESULTS: 82.0% of the teachers stated they had received orientation and 92.0% stated they had read the "Teacher Orientation Manual". Among those who received orientation, 47.0% stated it had been supplied by school officials; 30.0%, by healthcare agents, and 23.0% by ophthalmologists. In the self-assessment of performance, 58.0% reported no difficulties; 32.0% reported questions concerning the campaign activities and the professionals who most frequently asked to provide clarification were those from the State Education Agency (38.0%), followed by the school director (20.0%). CONCLUSIONS: Most teachers considered themselves adequately trained and oriented to take part in the campaign. Teachers' training was provided by administrative officials, previously trained by ophthalmologists ("multiplying effect") and the small percentage of questions raised indicated the validity of the procedure. Concerns have been raised as to the information about the ophthalmologic examination, transportation of the children and delivery of glasses. This kind of program always presents some difficulties and requires adjustments. It should, indeed, be improved, perhaps through a closer involvement of the community and families during its development. -

Post-visit at ophthalmology emergency service: frequency and perception of the doctors on duty and users
Publicationby Carvalho,Regina de SouzaOther Authors: Kara-José,Newton; Kara-Junior,NewtonOrigin: OasisbrPURPOSE: To check the occurrence of post-visit, the patient's understanding and the assessments of the doctor on duty about the efficacy of the process, at the ophthalmology emergency sector. METHODS: It was conducted a transversal, analytical research among doctors on duty and patients being seen consecutively in the ophthalmology emergency sector of Hospital das Clínicas da Universidade de São Paulo. RESULTS: The sample was made of 28 doctors on duty and 561 patients, 51.3% male and 48.7% female, with a mean age of 39.8 years old. From 34.1% of the patients who were previously seen by other sectors, 8.4% looked for two services and 5.7% three or more services. In the previous visits, 56.9% of the patients mentioned that they did not receive any explanation about their diagnostic. From the patients seen at Hospital das Clínicas, Faculdade de Medicina da Universidade de São Paulo - FMUSP, 95.1% mentioned that the ophthalmologists explained the diagnostic and from those 84.0% understood what was explained. Among 40.4% of the patients who received medicine prescriptions in the previous services, 85.5% mentioned that they received explanation about its use and 82.9% followed the orientation. At Hospital das Clínicas - FMUSP 95.0% of the patients understood how and why to use the medicine. At the ophthalmologists point of view, more than 90.0% of the patients understood the diagnostic and the prescribed treatment. CONCLUSION: According to this research, for most of the patients, it was provided post-visit orientation and patients' understanding about the disease and the treatments proposed. -

Gross Domestic Product (GDP) per capita and geographical distribution of ophthalmologists in Brazil
Publicationby Carvalho,Regina de SouzaOther Authors: Diniz,Alice Selles; Lacerda,Fabrício Martins; Mello,Paulo Augusto de ArrudaOrigin: OasisbrPURPOSE: To assess the number of ophthalmologists in Brazil, their regional distribution, ophthalmologist/habitant ratio, and the relation between ophthalmologist and State Gross Domestic Product (GDP) per capita to aid public health policies. METHODS: An ecologic study was conducted. Data were obtained from the "Census 2011 Brazilian Ophthalmology Council", from "Demographic Census of Brazilian Institute of Geography and Statistics (IBGE) 2010 and from "Brazilian Regional Accounts, 2005-2009"- Ministry of Planning, Budget and Management - IBGE. RESULTS: The number of ophthalmologists in Brazil is 15,719. Considering the performance in more than one municipality, the number of ophthalmologists in service is 17,992, that is, one ophthalmologist for 10,601; the ophthalmologist/site ratio vary among the States from a minimum of 1/51,437 (Amapá) to a maximum of 1/4,279 (Distrito Federal). There is a correlation among State GDP per capita and the number of ophthalmologists/habitant: the higher the GDP per capita, the larger is the number of ophthalmologists acting in the State (p<0.0001). CONCLUSION: According to this study, there is no lack of Ophthalmologists in the country, but a distribution imbalance which leads to professional shortage in particular places. A higher concentration of ophthalmologists/inhabitants was noticed in States which the economic growth is higher, expressed by the GDP per capita. -

Validity of recall absent schoolchildren to free eye health projects
Publicationby Noma,ReginaOther Authors: Carvalho,Regina de Souza; Kara-José,NewtonOrigin: OasisbrPURPOSE: To analyze the results of recall absent schoolchildren to eye health projects. METHODS: Cross-sectional study. Visual screening was performed in schoolchildren attending 1st to 4th grades at public schools, from 7 to 10 years-old, to select and forward to complete ophthalmic evaluation. The projects were performed during weekends, at a public school, in the same municipality. Free transportation, food and eyeglasses were offered. A second opportunity of examination was offered to the students who were absent from the first call, with the same facilities. RESULTS: 51,509 schoolchildren had their vision tested, 14,651 (28.4%) were referred for ophthalmic examination. Of these, 8,683 (59.3%) attended the first call, 2,228 (37.3%) attended the recall and 25.5% of parents did not take their children to ophthalmic examination. The need for eyeglasses for children who attended the examination was 23.8% and 32.0% in the first opportunity and recall, respectively. The recall increased the coverage in 15.2% (59.3% to 74.5%). CONCLUSION: An expressive number of parents (25.5%) did not bring their children to be examined, even at a second opportunity of exam. The facilities offered: access, free examination, transportation and glasses. Children who were absent in the first opportunity and appeared at recall had a greater need for eyeglasses. Recall increased the coverage in 15.2% (59.3% to 74.5%) and it is not recommended when financial resources are limited. -

A study of pent-up demand in ophthalmology: Divinolândia Hospital/Unicamp
Publicationby Machado,Maria CeciliaOther Authors: Kara-José,Newton; Arieta,Carlos Eduardo Leite; Lourenço,José Leonardo Garcia; Carvalho,Regina de SouzaOrigin: OasisbrOBJECTIVE: To assess the waiting time for eye care identifying the number of patients with each complaint; to investigate how the waiting time may worsen the patient's condition; to check the screening of urgent cases for effectiveness; and to devise means of increasing the medical-surgical care capacity. METHODS: A retrospective descriptive survey was conducted using data obtained on 12 occasions during collaborative team visits to provide eyecare services. These initiatives were designed to decrease the waiting time and to treat urgent cases that occurred on each occasion; eyecare services were provided every Saturday, in the period from June to August 2006, in 16 cities of the region covered by Conderg (Consortium for the Development of the São João da Boa Vista Administrative Region). RESULTS: Referrals used 1,743 (87.1%) of the 2,000 places available. The most frequent diagnoses were refractive errors, with 683 cases, corresponding to 39.1% of the total, followed by cataracts, with 296 cases, corresponding to 20.9%. Of the 238 surgeries indicated, 54.6% were phakectomies. Thirty-five (2.0%) cases were considered urgent. CONCLUSION: The most common diagnoses made during the team visits to manage the excess demand for eyecare were refractive errors and cataracts, which, together, accounted for the majority of the cases. The Divinolândia Hospital has the necessary human and material resources to meet the demand left unattended by the local SUS network. Immediate referral of urgent cases by the primary units' screeners proved effective. -
