Anticoagulant therapy was for a long time, limited to the use of heparins and warfarins. Recently, has been introduced in the market new alternative antithrombotic to this therapy. The availability of an oral of FXa or thrombin inhibitors has provided relevant clinical results in assays for the prophylaxis of postoperative venous thrombosis. However, these drugs do not have the polypharmacology of heparins. In this context, the investigation of small molecules that mimic the functions of the sulfated glycosaminoglycans, without the drawbacks characteristic of large chain heparins, has emerged as an area of research alternative. In this work, the major therapeutic approaches for the treatment of venous thrombosis are exposed, and is given a special focus to the study of our research group in the search for small molecules active dual antiplatelet and anticoagulant. Keywords: Anticoagulant therapy, Warfarins, Sulfated glycosaminoglycans

Polypharmacy – a frequent situation found among the elderly, related to their physiological changes and multiple morbidities – increases the risk of adverse reactions, interactions, drugs misuse and drug-related problems. Knowing that non-steroidal anti-inflammatory drugs (NSAIDs) are widely used among the elderly, the aims of the present study were to assess the potential interactions that may occur between prescribed NSAIDs and other drugs and to create a list of recommendations for monitoring elderly patients taking NSAIDs, in order to prevent or minimize those negative outcomes. With this purpose, a retrospective study was performed at Centro de Saúde de Eiras (Coimbra, Portugal). The study involved the systematic analyses of the medication regimens of a sample of elderly patients selected from the elderly population of this primary health care unit. Eligible subjects were patients aging 65 or more years and at least with one NSAID included in their therapeutic schemes in the last three months. A total of 37 prescribed NSAIDs were found in the 29 patients of the sample. 125 potential NSAID-drug interactions were found: 2 minor and 123 moderate. Most interactions occurred between NSAIDs and anti- hypertensive drugs, such as diuretics (17.6%), angiotensin receptor blockers (14.4%), calcium channel block- ers (12.0%) and angiotensin converting enzyme inhibitors (8.8%). The prevalence of interactions between NSAIDs was 12.8%. These interactions can cause adverse effects with particular manifestations in the elderly, such as renal impairment and increased blood pressure. The pharmacist can develop a relevant role in services such as medication review, to identify these potential interactions, or pharmacotherapy follow-up, in the management of these situations when properly identified. Stresses the importance of positive interaction with General and Family Practice for the safe and efficiency of therapies.

IBD is a gastro-intestinal disorder marked with chronic inflammation of intestinal epithelium, dam- aging mucosal tissue and manifests into several intestinal and extra-intestinal symptoms. Currently used medical therapy is able to induce and maintain the patient in remission, however no modifies or reverses the underlying pathogenic mechanism. The research of other medical approaches is crucial to the treatment of IBD and, for this, it ́s important to use animal models to mimic the characteristics of disease in real life. The aim of the study is to develop an animal model of TNBS-induced colitis to test new pharmacological approaches. TNBS was instilled intracolonic single dose as described by Morris et al. It was administered 2,5% TNBS in 50% ethanol through a catheter carefully inserted into the colon. Mice were kept in a Tredelenburg position to avoid reflux. On day 4 and 7, the animals were sacrificed by cervical dislocation. The induction was confirmed based on clinical symptoms/signs, ALP determination and histopathological analysis. At day 4, TNBS group presented a decreased body weight and an alteration of intestinal motility characterized by diarrhea, severe edema of the anus and moderate morbidity, while in the two control groups weren’t identified any alteration on the clinical symptoms/signs with an increase of the body weight. TNBS group presented the highest concentrations of ALP comparing with control groups. The histopathology analysis revealed severe necrosis of the mucosa with widespread necrosis of the intestinal glands. Severe hemorrhagic and purulent exsudates were observed in the submucosa, muscular and serosa. TNBS group presented clinical symptoms/ signs and histopathological features compatible with a correct induction of UC. The peak of manifestations became maximal at day 4 after induction. This study allows concluding that it’s possible to develop a TNBS- induced colitis 4 days after instillation.

Although the medicinal use of Cannabis sativa derivatives is well known since antiquity, the study of their properties expanded recently with the discovery of an endogenous cannabinoid system, which comprises the endogenous cannabis-like ligands (endocannabinoids), the cannabinoid receptors (CB1 and CB2) and the enzymes involved in their metabolism. Since the discovery of the endocannabinoid system (ECS), the scientific community focused on research of its clinical use and achieved important findings during the last decade. In some countries, cannabis derivatives are a pharmacological option for appetite stimulation and pain treatment. However, the first ECS-based drug rimonabant (a CB1 antagonist), approved for the treatment obesity with associated risk factors, was withdrawn due to safety concerns. Nowadays, based on the growing evidences resulting from preclinical and clinical studies of ECS modulators, these drugs are currently pointed out as novel therapeutic approaches for several pathophysiological conditions. Here, we review the potential role of (endo)cannabinoid system in therapeutics and the recent designed strategies for the development of drugs that target this system.

    Imunoterapia anti-tumoral com células dendríticas

Dendritic cells (DCs) are professional antigen-presenting cells, which display an extraordinary capacity to induce and regulate T-cell responses. Because of their immunoregulatory capacities and because very small numbers of activated DCs are highly efficient in generating immune responses against antigens, DCs have been extensively used in clinical trials in order to elicit or amplify immune responses against cancer. While clinical trials provide evidence that dendritic cells vaccines are safe and elicit immunological responses in most patients, few complete tumor remissions have been reported. To improve the clinical efficacy, it is mandatory to design novel and improved strategies that can boost adaptive immunity to cancer helping to overcome regulatory T cells and allowing the breakdown of the immunosuppressive tumor microenvironment. The association of DCs vaccines with conventional chemo- and radiotherapy protocols could enhance DCs activation and antigen cross-presentation, selectively eliminating immunosuppressive cells, thus reverting the immunosuppression state caused by cancer, suggesting that relevant chemoimmunotherapy associa- tions could fully exploit DC capacity to trigger anticancer responses.

Although drugs are developed with therapeutics intentions, sometimes some substances are used for purposes other than those the pharmacologist had in mind. The word “drug” also involves psychoactive substances that can be used for medicinal or non-medicinal purposes in which it is included substances that are legal or illegal. The use of drugs for intentions other than their therapeutic purposes has become a growing public concern making their study very important from the toxicological standpoint. When alleged victims report that they were robbed or assaulted while incapacitated by a drug, toxicological chemical analysis is imperative to help substantiate the victim’s claim. These criminal offenses are known as drug facilitated crimes. The branch of science dedicated to toxicology studies has two application fields, forensic and analytical toxicology, that are interdisciplinary and that comprise all the scientific facts related with crimes committed through the use of drugs. Most often, the cases of crimes facilitated by drugs involve substances that have strong central nervous system depressant effects that are availed to easily render victims to robberies or sexual assaults. As new drugs are continuously being synthetized and new methods are used for drugging victims without their knowledge or consent it is important to develop new analytical methods that can assist in the investigation for figuring out all the details about this sort of crimes or enable ways of their prevention. For example, one way of drugging victims without being aware of is through drink spiking in social entertainment places, during parties, raves, etc. In these cases, the spiked beverages are the “crime weapons”. In this paper, it will be presented the relation of drug facilitated crimes and the toxicology science and also a review focused on the available analytical methods for the detection of adulterated beverages with drugs.

Nowadays, generic medicines are used to reduce the health expenses in Portugal. These medicines arise when the reference medicine patent expires and have usually a lower price in order to contribute to the sustainability of the National Health Service. The Portuguese market of generic medicines began to increase since 2003. This was the consequence of the policies implemented to promote generic medicines. The prices of generic medicines have been declining constantly over the years, with a decrease of 42 % only from 2011 to 2012. The average reimbursement of medicines has a tendency to decrease which increases the amount that the patients pay. In this context, pharmacies live a critical period because of the decrease of the profit margins that are in some cases negative. Due to these situations, the accessibility to medicines by patients is questioned, since Portugal live in an economic and social crisis accompanied by a potential decrease of the pharmaceutical services coverage.

Information related to adverse effects of antiretroviral therapy can determine non-compliance and future therapeutic options. Evidence based practice includes a focalized clinical question to obtain the better information assessed by the parameters sensitivity and specificity. The main purpose of the work is the optimization of a systematic methodology of research clinical information about adverse effects of antiretroviral therapy, in MEDLINE database, with their identification in the literature and the construction of different search strategies to subsequent comparison of sensitivity and specificity values. A generic search strategy developed to find information related to drug iatrogenicity was considered as a reference to the development of search studied strategies. The results show the known adverse effects of antiretroviral agents that are mainly found in case reports. As the first work that approaches two different concepts simultaneously, adverse effects of antiretro-viral therapy and evidence based medicine approach to search literature, it shows and reinforces the real need for more available information to a better assessment of its quality.
