This trial aimed at evaluating the deleterious effects of aeration time on nutritive value and other fermentative characteristics of sugarcane silage. A completely randomized design was used with three treatments and four repetitions per treatment. Fresh chopped sugarcane was exposed to aeration for 0, 4 or 8 hours, and ensiled soon after. After exposure, the material was ensiled in 12 laboratory silos (plastic buckets). Silos were opened 85 days after ensiling, when organic acids contents and chemical composition of silages were determined. Deviation of linearity (p < 0.05) was observed for aeration time on dry matter. A positive linear effect was observed (p < 0.05) on ADF, NDF and soluble carbohydrates content, but negative for ammoniacal nitrogen content and in vitro digestibility of dry matter. For organic acids content, deviation of linearity was observed on acetic acid, with the lowest content (1.5% of DM) observed after 8 hours of aeration, and a negative linear effect was observed for lactic and butyric acids, as well as for pH values. There were no effects on ethanol concentration, which remained very high (22% of DM), regardless of aeration time. Aerobic stability of silage worsened with the increase in aeration time.

ABSTRACT This paper presents an integral model to evaluate the impact of gas transfer on the hydrodynamics of bubble plumes. The model is based on the Gaussian type self-similarity and functional relationships for the entrainment coefficient and factor of momentum amplification due to turbulence. The impact of mass transfer on bubble plume hydrodynamics is investigated considering different bubble sizes, gas flow rates and water depths. The results revealed a relevant impact when fine bubbles are considered, even for moderate water depths. Additionally, model simulations indicate that for weak bubble plumes (i.e., with relatively low flow rates and large depths and slip velocities), both dissolution and turbulence can affect plume hydrodynamics, which demonstrates the importance of taking the momentum amplification factor relationship into account. For deeper water conditions, simulations of bubble dissolution/decompression using the present model and classical models available in the literature resulted in a very good agreement for both aeration and oxygenation processes. Sensitivity analysis showed that the water depth, followed by the bubble size and the flow rate are the most important parameters that affect plume hydrodynamics. Lastly, dimensionless correlations are proposed to assess the impact of mass transfer on plume hydrodynamics, including both the aeration and oxygenation modes.

Objectives: To describe the use of antenatal corticosteroid and clinical evolution of preterm babies. Methods: An observational prospective cohort study was carried out. All 463 pregnant women and their 514 newborn babies with gestational age ranging from 23 to 34 weeks, born at the Brazilian Neonatal Research Network units, were evaluated from August 1 to December 31, 2001. The data were obtained through maternal interview, analysis of medical records, and follow-up of the newborn infants. Data analysis was performed with the use of chi-square, t Student, Mann-Whitney, and ANOVA tests and multiple logistic regression, with level of significance set at 5%. Results: Treatment was directly associated with the number of prenatal visits, with maternal hypertension and with the antenatal use of tocolytic agents. Babies from treated pregnant women presented better Apgar scores at the 1st and 5th minute, reduced need for intervention in the delivery room and lower SNAPPE II. They were born with higher birth weight, longer gestational age and needed less surfactant use, ventilation, and oxygenation time. After multiple logistic regression, the use of antenatal corticosteroid independently improved birth conditions, decreased ventilation time, being related to increased occurrence of neonatal sepsis. Conclusions: The use of corticosteroid was associated with better prenatal care and birth conditions, better preterm evolution but higher risk of infection. Objective: To report on the use of sildenafil for pulmonary hypertension treatment of a newborn patient after cardiac surgery. Description: A female, full term newborn infant with diagnosis of double outlet right ventricle, pulmonary hypoplasia and subaortic ventricular septal defect, was submitted to Blalock surgery in the first week of life. In postoperative the newborn had pulmonary hypertension and persistent hypoxia, without response to nitric oxide, but with improved oxygenation after continuous intravenous infusion of prostaglandin E1. After several failed attempts to discontinue prostaglandin E1, oral sildenafil was used. There was a decrease in pulmonary vascular resistance with consequent oxygenation improvement and 48 hours later it was possible to discontinue prostaglandin E1 infusion. Comments: Sildenafil can be an alternative therapy for pulmonary hypertension, especially when there is no response to conventional therapy. Copyright © 2005 by Sociedade Brasileira de Pediatria.

BACKGROUND AND OBJECTIVES: Myotonic dystrophies are autosomal dominant neuromuscular diseases. Among them, myotonic dystrophy type 1 (MD1), or Steinert disease, is the most common in adults, and besides muscular involvement it also has important systemic manifestations. Myotonic dystrophy type 1 poses a challenge to the anesthesiologist. Those patients are more sensitive to anesthetics and prone to cardiac and pulmonary complications. Besides, the possibility of developing malignant hyperthermia and myotonic episodes is also present. CASE REPORT: This is a 39-year old patient with DM1 who underwent general anesthesia for videolaparoscopic cholecystectomy. Total intravenous anesthesia with propofol, remifentanil, and rocuronium was the technique chosen. Intercurrences were not observed in the 90-minute surgical procedure, but after extubation, the patient developed respiratory failure and myotonia, which made tracheal intubation impossible. A laryngeal mask was used, allowing adequate oxygenation, and mechanical ventilation was maintained until full recovery of the respiratory function. The patient did not develop further complications. CONCLUSIONS: Myotonic dystrophy type 1 presents several particularities to the anesthesiologist. Detailed knowledge of its systemic involvement along with the differentiated action of anesthetic drugs in those patients will provide safer anesthetic-surgical procedure.

During the first year of ageing, the dissolved oxygen, dry extract, total polyphenol index, low molecular weight compounds, colour parameters and sensory properties of wine brandies were analysed with regard to the kind of ageing system: 650-L wooden barrels (traditional system) and 40-L stainless-steel tanks with wood tablets, with and without oxygenation (alternative systems). The barrels and wood pieces were manufactured from Portuguese chestnut wood (Castanea sativa Mill.) with heavy toasting level. The quantity of tablets was calculated in order to reproduce the surface/volume ratio of a 650-L barrel. The oxygenation was made by the application of air with a specific device in the middle height of the tanks, after 60, 90, 120, 180 and 360 days of ageing. In the experimental conditions, the results obtained demonstrated that the chemical composition and the colour evolution of the wine aged brandies are closely dependent on the ageing system. The oxygen effect on the total polyphenol index and on the low molecular weight phenolic compounds found in the brandies aged in traditional and alternative systems is quite different. This aspect is of great importance since it is strongly related with the chemical changes that occur during the ageing process and determine the quality of the brandies. Contrary to what have been observed in the ageing of red wines, the oxygen consumption in the brandies aged in wooden barrels did not induced colour stabilization, suggesting that the chemical mechanisms and the compounds responsible for the colour changes of the brandy should be different from those identified in red wine.

Assessment of a Microfluidic Intravenous Oxygen Generating Platform to Aid Acute Respiratory Failure

    Publicação
    por Alimagham, Farah Christina
Acute respiratory failure is associated with a high mortality rate, despite the advances in conventional treatments. This work presents the development of a proof-of-concept device for assessing the viability of an oxygen-generating catheter, deployed intravenously, to temporarily sustain a patient who is suffering from acute respiratory failure. The assessment device mimics the interface between the catheter and bloodstream (deoxygenated water substitutes the blood), and consists of two parallel channels separated from each other by an oxygen-permeable membrane that simulates the catheter material. Several polydimethylsiloxane membranes with enhanced permeability were developed and tested on the device according to their permeation rates. The highest permeation rate achieved was 3.6×10-7 cm3/s (equivalent in-blood value) considering the device’s surface area and applied pressure. However, the extrapolation of this value to a catheter with increased surface area demonstrated a predicted oxygen permeation rate of 1.6×10-3 cm3/s. Although the oxygen permeation rates achieved here do not yet reach the minimum required rate to sustain a patient with only 30 % of their lungs functional (1.6 cm3/s O2), it may be enhanced further by improving certain parameters such as material permeability, surface area and applied pressure. The ability to administer oxygen or other gases directly into the bloodstream may portray a technique for short-term rescue of severely hypoxemic patients to increase whole body or at-risk organ oxygenation.
