Study on erythropoietin subconjunctival administration in a glaucoma animal model

por Resende, Ana Paula Simões Nunes de
Glaucoma is the number one cause of irreversible vision loss worldwide. Death of retinal ganglion cells (RGC), which results in the progressive loss of visual function, occurs in glaucoma and other ocular diseases caused by hypoxia and ischemia. Although glaucoma is a multifactorial neurodegenerative disease, the only currently method of treatment involves reduction of intraocular pressure and, at the present, there is no effective treatment to prevent RGC apoptosis. Notably, it has been reported that erythropoietin (EPO), a cytokine hormone produced in response to hypoxia, has significant neuroprotective and neuroregenerative properties in several types of ocular disorders. Pre-clinical studies in glaucoma animal models involving EPO have yielded very promising results. All studies involving EPO ocular administrations have used systemic, intravitreal or retrobulbar administration to reach retinal desired EPO concentrations. However, EPO chronic systemic administration can lead to adverse side effects related with haematopoiesis stimulation, while intravitreal or retrobulbar administrations are invasive procedures that can induce several complications such as endophthalmitis, retinal detachment, vitreitis, retinitis, choroiditis or cataracts. This thesis aims to clarify if EPO’s neuroprotection could be achieved by a non-invasive and safe periocular administration route without adverse effects. Being so, this work evaluates the subconjunctival route as an alternative for EPO administration in glaucoma disease. After the first in vitro study, where the permeation of EPO across the periocular tissues was quantified, all work was developed in in vivo models. EPO’s ocular permeation after subconjunctival administration was tested, both in physiological and glaucomatous conditions. Furthermore, both retinal morphological and physiological effects of EPO administered by this route were assessed in glaucomatous animals. Results showed that EPO, when administered subconjunctivally, can permeate the main ocular barriers and reach RGC layers, in both physiological and glaucomatous conditions, without significant local or systemic side effects. More than showing that EPO can reach the retina by this route, results also concluded that subconjunctival EPO administration seems to have structural and functional beneficial effects on the retina after glaucoma induction in rats.

The present work investigates the effects of chitosan-hyaluronic acid-epoetin beta (CS/HA-EPOβ) nanoparticles after topical ocular administration in a rat glaucoma model. Wistar Hannover rats (n = 24) were submitted to a complete ophthalmological examination and electroretinography, followed by glaucoma induction in their right eye on day 1 of the study. Treatment group (T) received CS/HA-EPOβ nanocarriers (n = 12), while the control group (C) received only empty ones. Electroretinography was repeated on day 3 (n = 24) and before euthanasia on day 7 (n = 8), 14 (n = 8), and 21 (n = 8), followed by bilateral enucleation and histological assessment. The animals showed good tolerance to the nanoformulation. Maximum IOP values on the right eye occurred shortly after glaucoma induction (T = 62.6 ± 8.3 mmHg; C = 63.6 ± 7.9 mmHg). Animals from the treated group presented a tendency for faster recovery of retinal electrical activity (p > 0.05). EPOβ was detected on the retina of all treated eyes using immunofluorescence. Control animals presented with thinner retinas compared to the treated ones (p < 0.05). Therefore, topical ocular administration of CS/HA-EPOβ nanoparticles enabled EPOβ delivery to the retina of glaucomatous rats and promoted an earlier retinal recovery, confirming EPOβ’s neuroprotective effects. The encouraging results of this preclinical study pave the way for new strategies for topical ocular administration of neuroprotective compounds.

An increasing incidence of eye diseases has been registered in the last decades in developed countries due to the ageing of population, changes in lifestyle, environmental factors, and the presence of concomitant medical conditions. The increase of public awareness on ocular conditions leads to an early diagnosis and treatment, as well as an increased demand for more effective and minimally invasive solutions for the treatment of both the anterior and posterior segments of the eye. Despite being the most common route of ophthalmic drug administration, eye drops are associated with compliance issues, drug wastage by lacrimation, and low bioavailability due to the ocular barriers. In order to overcome these problems, the design of drug-eluting ophthalmic lenses constitutes a non-invasive and patient-friendly approach for the sustained drug delivery to the eye. Several examples of therapeutic contact lenses and intraocular lenses have been developed, by means of different strategies of drug loading, leading to promising results. This review aims to report the recent advances in the development of therapeutic ophthalmic lenses for the treatment and/or prophylaxis of eye pathologies (i.e., glaucoma, cataract, corneal diseases, or posterior segment diseases) and it gives an overview of the future perspectives and challenges in the field.

Unravelling the role of Piezo1 channel in elevated pressure-exposed microglia: Effect on oxidative stress, proliferation and antigen presentation.

    Publicação
    por Carvalho, Catarina Gonçalves
Glaucoma is a neurodegenerative disease, characterized by vison loss derived from the degeneration of the optic nerve and retinal ganglion cell (RGCs) death. Previous works have shown that neuroinflammation speared by microglia is major part in the pathology. Also, published work from our group has shown that cytotoxic factors involved in the death of retinal cells have originated from microglia exposed to an elevated hydrostatic model. Not only that, but it has been shown that controlling microglia reactivity showed a positive impact on the RGCs both in vitro and in vivo. However, the sensor for this pressure changes has yet to be identified. Piezo1, an ion channel sensitive to mechanical stimuli, is responsible for the detection of various types of pressure changes, such as sheer stress and stretching. It has been shown to be expressed in the microglia and to be involved in inflammation, yet the study of this channel in the retina remains to be addressed. So, we aimed to investigate the role of Piezo1 in microglia-induced neuroinflammation in glaucoma. BV-2 cells were exposed to elevated hydrostatic pressure for 4 or 24 hours in the presence or absence of 1µM of GsMTx-4, a mechanosensor inhibitor. Also, the cells were exposed to 10µM of Yoda-1, a Piezo1 selective agonist also in the presence or absence of GsMTx-4. This work was divided in two main tasks. The first task’s role was to understand the role of Piezo1 in elevated pressure induced microglia reactivity, and it was divided in two main paths: assessing ROS production using DHE, MitoSox Red and DCF, probes designed for various ROS and cell proliferation was analysed using Click-iT EdU reaction. The second task consisted in understanding the role of Piezo1 in phenotype transition of microglia and for that we used a fluorescence probe, DQ-Ovalbumin, to evaluate antigen processing. Overall, the results of ROS production show no statistical differences, only apparent trends, such as the decrease of ROS production when GsMTx-4 is used in tandem with the stimuli. The cells appear to show a significant decrease of proliferation when Yoda-1 is used for 24 hours, fact that it is not derived from possible cell death or metabolic problems, otherwise all the other experimental conditions show no statistical changes. BV-2 cell’s antigen processing seems to not be affected in the overall, showcasing no significant changes, however, the exposure of these cells to EHP for 24 hours seem to indicate a trend. This work showed that despite the overall lack of significant changes, some trends are shown, and more work is needed to explore them. In the end, this work leaves many doors open for future research and a better understanding of Piezo1 and microglia-induced neuroinflammation

Role of Piezo1 in microglia-mediated neuroinflammation: Dissecting the role of Piezo1 activation in the retina

Activation of a3 adenosine receptor using intraocular biodegradable implants as a strategy for the treatment of glaucoma

    Publicação
    por Boia, Raquel Sofia Freitas
Glaucoma is a leading cause of irreversible blindness that is characterized by optic nerve damage and retinal ganglion cell (RGC) death. Elevated intraocular pressure (IOP) is an important risk factor in glaucoma and the only that is modifable. Indeed, current treatments are directed towards IOP lowering, however many patients continue to lose vision despite successful IOP control. Therefore, new and more effective treatments are necessary, and targeting neuroprotection of RGCs is considered to be an additional therapy. RGCs express adenosine A3 receptor (A3R) and its activation confers protection to RGCs following an excitotoxic stimulus, as occurs in glaucoma. These fndings strongly support that A3R activation can protect RGCs from glaucomatous damage. Therefore, we aimed to study the protective properties of A3R agonist against glaucomatous insult (Chapter 2). Ocular hypertension (OHT) was induced by laser photocoagulation of the limbal veins and A3R agonist (2-Cl-IB-MECA, 5 µl, 1.2 µM) was delivered intravitreally immediately after the induction of OHT. The outcome was assessed 7 days post injection. The treatment with A3R agonist increased RGC survival and attenuated the impairment in retrograde axonal transport induced by OHT, which is consistent with the preservation of the optic nerve structure. These benefcial effects of A3R activation may be contributing to the maintenance of the RGC function in OHT animals. Therefore, A3R can be suggested as a good therapeutic target to protect RGCs from glaucomatous damage. Drug delivery into the posterior part of the eye is mainly achieved by intravitreal injection. However, frequent intravitreal injections can lead to retinal detachment, cataract, and endophthalmitis. Therefore, the design of sustained drug delivery systems for the posterior segment of the eye can encompass a therapeutic breakthrough. The development of these systems has been a challenge for many years, but it is nowadays used in clinical practice for some retinal diseases. Biodegradable implants present an additional advantage since they are degraded into nontoxic products and, then safely eliminated, contrary to nonbiodegradable implants. Aiming to overcome the need of multiple intravitreal injections, herein a new biodegradable intraocular implant (porous poly(Ɛ-caprolactone) (PCL)-based intraocular implants) was developed for controlled drug release in collaboration with the Chemical Engineering Department of the Faculty of Sciences and Technology of the University of Coimbra (Chapter 3, supplementary information). Thus, the potential of PCL-based implants to be used as intraocular drug delivery device was assessed (Chapter 3). The presence of the PCL implant or the degradation products did not cause neuronal cell death, particularly did not affect the number of RGCs, in retinal primary neural cell cultures and retinal organotypic cultures, reinforcing the possibility of testing these implants in animal models. The PCL-based implants were surgically introduced in the vitreous of Wistar rats, and the outcome was evaluated at 4 and 8 weeks after the surgery. The presence of the PCL implant or the procedure did not change retinal function (evaluated by electroretinography, ERG) nor retinal structure (evaluated by optical coherence tomography, OCT). Moreover, PCL implant did not induce alterations in retinal neurons and had no toxic effect in RGCs in vivo, since the number of RGCs was not altered. The impact to retinal glial cells was assessed, and both the procedure and the presence of PCL implant may induce Müller cell gliosis, without affecting microglial cells and astrocytes. The assessment of retinal structure18 and Müller cell reactivity 1 year after surgery demonstrated that the structure was not altered and that Müller cell gliosis observed in early time points was a transient effect. Taking into consideration the lack of retinal toxicity of PCL implants, this new device can now be loaded with drugs to tackle retinal diseases. Indeed, since the activation of A3R confers protection to the retina, in particular to RGCs, implants loaded with 2-Cl-IB-MECA can be hypothesized as a new strategy to protect the retina from degeneration (Chapter 4). 2-Cl-IB-MECA-loaded PCL implants were developed in collaboration with the Chemical Engineering Department of the Faculty of Sciences and Technology of the University of Coimbra (Chapter 4, supplementary information). The PCL implant loaded with 2-Cl-IB-MECA presented an extended drug release of around 30 days. Then, the drug released from the implant maintained functional activity, as determined in vitro by single-cell Ca2+ imaging of RGCs. 2-Cl-IB-MECA-loaded PCL implants were immersed in a saline solution for 24 h that was able to reduce the glutamateevoked increase in intracellular Ca2+ concentration in immunopurifed RGC cultures. Drug-free and 2-Cl-IB-MECA-loaded PCL implants were introduced in the vitreous after inducing transient retinal ischemia, and the outcome was assessed 1 month after lesion induction. The presence of 2-Cl-IB-MECA-loaded PCL implants in the vitreous preserved the structure of the optic nerve and preserved the anterograde axonal transport. Moreover, the survival of RGCs was increased and the function of RGCs was maintained upon transient retinal ischemia, indicating that RGCs are protected from ischemia-reperfusion damage. These results suggest that A3R represents a potential drug target for the development of novel therapeutic strategies for glaucoma. Moreover, the incorporation of 2-Cl-IB-MECA in a biodegradable intraocular implant may be a promising therapeutic strategy for glaucoma.

Influence of elevated hydrostatic pressure on Müller cells phenotype. Do microglia-derived microvesicles play a role?

The visual impairment that often leads to blindness causes a higher morbidity rate. The goal of this work is to create a novel biodegradable polymeric implant obtained from coaxial fibers containing the dispersed drug-acetazolamide-in order to achieve sustained drug release and increase patient compliance, which is of the highest importance. Firstly, during this work, uncoated implants were produced by electrospinning, and rolled in the shape of small cylinders that were composed of uniaxial and coaxial fibers with immobilized drug inside. The fibers were composed by PCL (poly ε-caprolactone) and Lutrol F127 (poly (oxyethylene-b-oxypropylene-b-oxyethylene)). The prepared implants exhibited a fast rate of drug release, which led to the preparation of new implants incorporating the same formulation but with an additional coating film prepared by solvent casting and comprising PCL and Lutrol F127 or PCL and Luwax EVA 3 ((poly (ethylene-co-vinyl acetate)). Implants were characterized and in vitro release profiles of acetazolamide were obtained in phosphate buffered saline (PBS) at 37 °C. The release profile of the acetazolamide from coated implant containing Luwax EVA 3 is considerably slower than what was observed in case of coated implants containing Lutrol F127, allowing a sustained release and an innovation relatively to other ocular drug delivery systems.
