Consumo de álcool de mulheres em um serviço de Atenção Primária à Saúde*
Publicaçãopor Ponce, Talita DutraOutros Autores: Picciano, Andressa Pelissoli; Vargas, Divane deOrigem: OasisbrObjective: To identify alcohol consumption patterns in people cared by a Primary Health Care service and verify the association between the patterns and the variables of the sample. Method: Our observational study used both the Alcohol Use Disorders Identification Test instrument and a questionnaire with socio-demographic, clinical, and behavioral questions for data collection. We carried out the descriptive and univariate analysis with Kendall and Kruskal-Wallis correlation tests. We introduced variables with p ≤ 0.2 values in the multiple logistic regression – Mann-Whitney test. Results: The sample of the study was constituted by 561 women. The analysis results indicated relevant influence for higher patterns of consumption: not having a partner, not having a religion, smoking and drug habits, and having arterial hypertension. Besides that, within each additional year in women’s age, the alcohol consumption decreases. Conclusion: We endorsed data that female alcohol consumption is presenting a tendency to increase especially among younger women, this information is essential for the promotion of Primary Health Care. -

Screening of recently seized new psychoactive substances in urine of potential consumers and urban wastewater
Publicaçãopor Celma, AlbertoOutros Autores: Bijlsma, Lubertus; Sancho, Juan V.; Hernandez, Felix; Dias, Mário; Simões, Susana et al.NewPsychoactive Substances (NPS) areconstantly being developed as legalsubstitution of traditional drugs of abuse.Although the traditional drugs maintain popular, new drugs are regularly introduced, changing the drug market ceaselessly. Monitoring NPS and its consumption is challengingas these compoundsare normally missed in routine drug analysis. Users do often not exactly know what they consume andinformation available islimitedof what is being sold. Furthermore, very little information exists onmetabolism of these newly introduced NPS. This highlights the needof applying a strategic workflow making use of modernanalytical techniques to face this novel public health safety challenge. The workflowpresented in this work consists of three stages: i)the creation of a databaseincluding approximately 200 NPS. These NPS wererecently reported to the National Early Warning Systems (EWS) ofSpain andItaly and the EWS of the European Monitoring Center for Drugsand Drug Addiction (EMCDDA). ii) The collection of urine samples of individuals suspected of drug consumption, pooled urine samples from festivals and urban wastewater samples. iii) The screening of NPS using ultra-high performance liquid chromatography coupled to high resolution massspectrometry with a hybrid quadrupole time-of-flight mass analyzer. Although the database is under constant development, including NPS reported to be present in wastewaters around Europe, new compounds appearing in the market, as well as metabolites reported in the literature, the workflow applied allowed the detection of several unchanged NPS in urine and pooled urine samples. In several cases, NPSwere also detected in wastewater samples, which indicatesthat consumptionof some of these compounds is elevated. The absence of reference standards for several NPS found, as well as for some NPS metabolites, made it unfeasible the full confirmation of some compounds in the urine/wastewater samples analyzed. However, accurate-mass full-spectrum data provided by HRMS allowed their tentative identification with high degree of reliability. The subsequent acquisition of reference standards, when available, will allow the unequivocal confirmation of their identity. -

MDMA intoxication in a potential organ donor with cardiac arrest
Publicaçãopor Castro, André LoboOutros Autores: Tarelho, Sónia; Almeida, Dina; Sousa, Lara; Franco, João; Teixeira, Helena M.Amphetamine and its derivatives’ consumption is still an important public health issue, namely in terms of compounds variability and disposition to consumers. However, some of them, like MDMA, still live in the illicit market, with continuous success. Nevertheless, there is always new information and data on MDMA intoxication, both in vivo and in post-mortem context. The authors report an intoxication case with MDMA, in an 18 years old male, considered a potential organ donor after a cardiac arrest. Whole blood samples were collected in vivo, at the Emergency Room (ER), and post-mortem, at the National Institute of Legal Medicine and Forensic Sciences. After a general screening procedure, samples were extracted by SPE (OASIS® MCX), followed by GC-MS analysis. The whole blood post-mortem sample was positive for lidocaine (< 500 ng/mL), compatible with the ER intervention, and positive for MDMA (2278 ng/mL) and MDA (49 ng/mL), while whole blood samples collected in vivo (during the maintenance of the individual under advanced life support), were positive for MDMA (504 ng/mL to 1918 ng/mL) and MDA (20 ng/mL to 89 ng/mL). Samples were negative for other substances, namely ethanol, other drugs of abuse and medicines. Results interpretation is pivotal to understand the behaviour of the substance. Thus, in this case, MDMA post-mortem behaviour should be carefully evaluated, considering as possible influencers, in the specific context of the case, the time lapse between death verification, maintenance of the advanced life support and body manipulation for organ collection purposes. Also referred and discussed is the ante-mortem/post-mortem ratio of MDMA obtained values, compared with literature references. There is no doubt that death was due to MDMA intoxication, but information from the analysis performed on the in vivo samples suggests that this type of sample should also be considered, in a complementary role, whenever possible. -

Detection and quantification of selected cannabinoids in oral fluid samples by protein precipitation and LC-MS/MS
Publicaçãopor Antunes, MónicaOutros Autores: Simões, Susana; Fonseca, Suzana; Franco, João; Gallardo, Eugenia; Barroso, MárioCannabis is the most widely consumed illicit drug worldwide. As consumption rates increase, partially due to the decriminalization of its use for medicinal and recreational purposes, analytical methods for monitoring different cannabinoids in several biological matrices have been developed. Herein, a simple and fast extraction procedure to extract natural cannabinoids from oral fluid (OF) samples was developed and fully validated according to the ANSI/ASB 2019 Standard Practices for Method Validation in Forensic Toxicology. Using only 0.2 mL of neat OF, the analytes [Δ9-tetrahidrocannabinol (THC), 11-hydroxy-Δ9-tetrahydrocannabinol (THC-OH), 11-nor-9-carboxy-Δ9-tetrahydrocannabinol (THC-COOH), cannabinol (CBN) and cannabidiol (CBD)] were extracted by protein precipitation with a mixture of methanol:acetonitrile (80:20, v/v); the extracts were centrifuged, evaporated to dryness and reconstituted in 100 µL of methanol. Analysis was performed by liquid chromatography coupled to tandem mass spectrometry (LC–MS/MS). The developed methodology produced linear results for all compounds, with working ranges of 0.1–50 ng/mL for THC, 0.5–50 ng/mL for THC-OH, CBN and CBD, and 0.05–1 ng/mL for THC-COOH. Ion suppression was observed for THC, CBN and CBD, which did not impair sensitivity considering the low limits of quantification (LOQs) and limits of detection (LODs) obtained (which varied between 0.05 and 0.5 ng/mL). The extraction procedure produced great recoveries, and the compounds were stable. No interferences were found, and the method proved to be extremely fast, selective, precise, and accurate for use in routine analysis. The method was successfully applied to authentic samples. -

The drugs phenomenon from the perspective of nursing students: patterns of consumption, attitudes and beliefs
Publicaçãopor Magalhães,Luciana de Souza Pereira deOutros Autores: Vernaglia,Taís Verônica Cardoso; Sousa,Flávia Abrahão Marcolan de; Chagas,Silvana Vieira da; Cruz,Marcelo SantosOrigem: OasisbrAbstract Objective: To describe and analyze patterns of drug use and abuse of first- and fifth-year undergraduate nursing students and to investigate their attitudes and beliefs regarding drugs and users. Method: A quantitative, descriptive and cross-sectional study of 160 students from the Federal University of the State of Rio de Janeiro was performed. Participants were interviewed using the instruments: ASSIST and NEADA FACULTY SURVEY. Collected data were analyzed with significance being set for p-values < 0.05. Results: Students of the first and the fifth years reported that alcohol had been the most frequently used drug in the previous three months (69.4% and 80.0%, respectively). Students believe they have adequate basic education about drugs however they present a prejudiced view and negative attitudes toward users. Conclusion: The pattern of drug use among students and the lack of education about the care of drug users reinforce the need to review and reformulate course contents and practices on the subject. -

Chiral resolution and enantioselectivity of synthetic cathinones: A brief review
Publicaçãopor Silva B.Outros Autores: Fernandes C.; de Pinho P.G.; Remião F.Chiral discrimination has become one of the most important fields in analytical and medicinal chemistry, and forensic toxicology. The enantiomers may have different binding to proteins that may lead to many pharmacological and toxicological differences between them, including kinetic (at the absorption, distribution, metabolism and excretion level) or dynamic (level of potency and efficacy or even differences in mechanism of action) variations. Cathinone derivatives are chiral compounds, sold via the internet for recreational use, and little is known about their enantiomeric selectivity. Consequently, it is of crucial importance for the development of resolution methods to obtain pure enantiomers to study their biological effects. In the last few years, techniques for chiral drug analysis, as chromatography, have been developed and some works related to the analytical enantiomeric resolution of synthetic cathinones were described. However, information about synthetic cathinones in the literature is scarce specially concerning single enantiomers. In this mini-review, analytical chiral resolution and biological differences between enantiomers of cathinone derivatives will be addressed. © The Author 2017. Published by Oxford University Press.
