Inverse dynamics analysis as a complement of shoulder physical therapy assessment

Bone remodelling of the humerus after a resurfacing and a stemless shoulder arthroplasty
Publicaçãopor Santos, B.Outros Autores: Quental, Carlos; Folgado, João; Sarmento, Marco; Monteiro, JacintoBackground: New implant designs, such as resurfacing and stemless implants, have been developed to improve the long-term outcomes of the shoulder arthroplasty. However, it is not yet fully understood if their influence on the bone load distribution can compromise the long-term stability of the implant due to bone mass changes. Using three-dimensional finite element models, the aim of the present study was to analyse the bone remodelling process of the humerus after the introduction of resurfacing and stemless implants based on the Global C.A.P. and Sidus Stem-Free designs, respectively. Methods: The 3D geometric model of the humerus was generated from the CT data of the Visible Human Project and the resurfacing and stemless implants were modelled in Solidworks. Considering a native humerus model, a humerus model with the resurfacing implant, and a humerus model with the stemless implant, three finite element models were developed in Abaqus. Bone remodelling simulations were performed considering healthy and poor bone quality conditions. The loading condition considered comprised 6 load cases of standard shoulder movements, including muscle and joint reaction forces estimated by a multibody model of the upper limb. Findings: The results showed similar levels of bone resorption for the resurfacing and stemless implants for common humeral regions. The regions underneath the head of the resurfacing implant, unique to this design, showed the largest bone loss. For both implants, bone resorption was more pronounced for the poor bone quality condition than for the healthy bone quality condition. Interpretation: The stemless implant lost less density at the fixation site, which might suggest that these implants may be better supported in the long-term than the resurfacing implants. However, further investigation is necessary to allow definite recommendations. -

The pectoralis minor muscle and shoulder movement-related impairments and pain: Rationale, assessment and management
Publicaçãopor Morais, NunoOutros Autores: Cruz, JoanaOrigem: IC-onlineThe adaptive shortening or tightness of the pectoralis minor muscle (PMm) is one of the potential biomechanical mechanisms associated with altered scapular alignment at rest and scapular motion during arm elevation (scapular dyskinesis) in patients with shoulder complaints. This masterclass briefly reviews the role of the PMm in shoulder movement-related impairments and provides a critical overview of the assessment of PMm tightness and the conventional approaches to increase its resting length and extensibility. A rehabilitation approach focused on PMm stretching and simultaneous optimization of the kinematic chain of arm elevation is also discussed, hoping to improve the management of shoulder movement-related impairments and pain. -

Cross-cultural adaptation and validation of the european portuguese version of the western ontario shoulder instability index (WOSI)
Publicaçãopor Torres, Rui ManuelOutros Autores: Faria, Sara Isabel; Cavalheiro, Luís Manuel; Lopes Ferreira, Pedro; Soles Gonçalves, RuiIntroduction: The Western Ontario Shoulder Instability Index (WOSI) is a self-administered questionnaire specifically used to determine the impact of shoulder instability on quality of life. The aim of this study was to translate the WOSI into European Portuguese and analyze its validity and reliability in a population with shoulder instability.Material and Methods: The WOSI was translated and culturally adapted from its original version into European Portuguese (WOSI PT). Internal consistency and test-retest analyses were conducted to determine the level of reliability of the scale. WOSI-PT, Quick DASH, and SF-12 questionnaires were applied to 81 patients with symptomatic shoulder instability to assess validity, and reliability was tested by randomly selecting 50 patients within 72 hours using a test-retest design. Results: The reliability of the WOSI-PT was very high, with Cronbach´s alpha equal to 0.97 and an intraclass correlation coefficient of 0.98. Regarding the construct validity, the correlation between the WOSI-PT and QuickDASH was high and negative (-0.79). The correlations between WOSI-PT and SF-12 were positive, respectively, moderate with physical (0.66) and low with mental (0.34) health. Conclusion: WOSI-PT is a reliable and valid instrument for assessing the functional impact of shoulder joint instability on quality of life. -

Design and development of a new medical device to support the diagnosis of the shoulder instability
Publicaçãopor Vilaça, Paula Cristina GonçalvesThe shoulder joint is the most movable joint of the human body. Due to its wide range of motion, the shoulder joint is also the most frequently dislocated joint in the human body. Once, a joint is dislocated its stability becomes affected which compromises its normal function, making it more difficult to realize the normal daily activities. The diagnosis of shoulder joint lesions is realized in two different steps: firstly, the physical exams, realized by the healthcare professional, with the aim of evaluating the joint function, through its palpation, as well as, the surrounding area. Moreover, in this exam, the motion of the joint is also simulated, moving it in different directions, in order to perceive abnormal motions of the joint. Secondly, the diagnosis is realized through an imaging exam, where is evaluated the position of the bones and, in case of some imaging exams, the condition of the structures which enfold the bones. The realization of the two aforementioned exams is performed in different instants in time conducting, sometimes, to divergent results, which can lead to an incorrect diagnosis of the patient condition. Furthermore, these diagnosis techniques do not allow the accuracy and the reproducibility of the results, since that, they rely on the subjective interpretation of the healthcare professional. Hence, in order to improve the quality of the diagnosis as well as to facilitate its process, the project design of a medical device to support the diagnosis of shoulder instabilities is proposed in the present dissertation. The development of the present project is divided in several stages, since the creation of a set of concepts to the medical device, to the validation of the final selected solution. Previously to the creation of concepts it was realized a survey to understand the customer needs. These were translated into functions, which device should realize, and a set of requirements, which medical device should contemplate. The concepts to the medical device were three-dimensional modulated to allow a clear comprehension of the respective functionality. The final solution was selected through a comparison evaluation between the concepts, using a set of selection criteria. Then, this final solution was improved, and it was validated. The final adopted solution possibilities the association, at the same time, of the shoulder joint movements with an imaging exam, like Magnetic Resonance Imaging. In this sense, the present medical device becomes the diagnosis more objective, allowing a more precise evaluation, which improves the quality of medical treatment. -

Evaluation of rotator cuff muscle strength in healthy individuals
Publicaçãopor Cortez,Paulo José OliveiraOutros Autores: Tomazini,José EliasOrigem: OasisbrOBJECTIVE: To compare the strength generated by the rotator muscles of the shoulder joint between the right upper limb and left upper limb among healthy individuals. METHODS: To evaluate the muscle strength of upper limbs from isometric contractions in the horizontal direction (rotation) an isometric dynamometer was used, equipped with transducers, signal conditioning, a data acquisition board, and finally, a computer. Study participants were 22 male military subjects, aged between 18 and 19 years old, body mass between 57.7 and 93.0 kg (71.8 ± 9.45 kg) and height between 1.67 and 1.90 m (1.75 ± 0.06 m), healthy and without clinical diseases or any type of orthopedic injury in the muscle skeletal system. RESULTS: The internal rotation in the right upper limb (RUL) was higher than the average strength of internal rotation in the left upper limb (LUL) (p = 0.723). The external rotation strength in RUL was lower than the average strength of external rotation in the LUL (p=0.788). No statistical difference was observed by comparing the strength values of all isometric strength tests. CONCLUSION: For the sample and methodology used to assess muscle strength, there was no statistical difference between the strength generated by the muscles of the rotator cuff of the right and left upper limbs. Publicaçãopor Rezende,Bruno da Rocha MoreiraOutros Autores: Almeida Neto,José Inácio de; Sousa,Uriel Jaime de; Bomfim,Leônidas de Souza; Ferreira Júnior,Mário SoaresOrigem: OasisbrOBJECTIVE: To evaluate and to compare the Spaso and Kocher reduction maneuvers in terms of efficiency, time of reduction, facileness, pain, discomfort to the patient, complications and risks, besides promoting the comparison between the two maneuvers, aiming to ground the emergency conduct. METHODS: A prospective study with 105 patients with acute shoulder dislocations were enrolled in the study between February 2011 and September 2012. The patients were randomized into two groups and they were submitted to the reduction of dislocation using the Spaso maneuver (group A) or the Kocher technique (group B) by first, second or third-year orthopedic surgery residents from our service. The results were evaluated and compared. RESULTS: There was no statistically significant difference between the two groups in terms of age, gender, timing of reduction, number of precedent episodes and complications after reduction. However, reduction was achieved in more cases using the Spaso method than it was among the Kocher group, as well as the mean duration of the reduction maneuver and discomfort were shorter in the group A patients. CONCLUSIONS: Both methods presented good results in terms of dislocation reduction and low complications rates. Nevertheless, the Spaso maneuver was more efficient, fast and easily applicable in comparison with Kocher's method. Level of Evidence I, Therapeutic. -
