Older adults with dementia: knowledge and attitudes of physicians in health units

Burnout in Family Doctors from a Portuguese Region in Relation to the COVID-19 Pandemic
Publicaçãopor Camões, Ana CatarinaOutros Autores: Melo Ferreira, Maria Inês; Fróis, Ana Teresa; Silva, José Caetano; Costa, Cassilda; Correia, Raquel S. et al.Origem: Gazeta MédicaINTRODUCTION: During the COVID-19 pandemic, burnout cases appear to have worsened within physicians. This study aimed at determining the prevalence of burnout, reflecting the COVID-19 pandemic, in family physicians from a region in Portugal.MATERIAL AND METHODS: Analytical cross-sectional study with a self-administered online questionnaire, during april and march of 2021. Application of Maslach Burnout Inventory-Health Services Survey.RESULTS: 60 valid responses were obtained, with a mainly feminine (82%) sample, between 36 and 45 years of age (53%). A total of 52% claimed to have thought of changing jobs, institution, or service in the past month. A percentage of 70% presented high emotional exhaustion, 40% high depersonalization, and 45% low personal realization. Suffered from burnout 32%. Emotional exhaustion was higher for those with weekly schedule above 40 hours (p=0.011). Low personal realization (p=0.004) and burnout (p=0.034) were more prevalent in the lower age groups. There were no significant relations between burnout and sex or place of work.DISCUSSION: Burnout estimated prevalence in family physicians is twice that of 2016 in the north of Portugal and eight times that of 2012 nationwide. Given the temporal relationship, there is probably a relation between this rise in prevalence and the COVID-19 pandemic. The higher prevalence of burnout in the younger physicians may be due to less effective coping strategies against work stress.CONCLUSION: Burnout prevention should be a priority within family physicians, given its high prevalence and impact on mental health, as well as being a form of quinquinary prevention in health. -

Implementing alcohol screening and brief interventions in primary health care: study protocol for a pilot cluster randomized controlled trial
Publicaçãopor Rosário, FredericoOutros Autores: Vasiljevic, Milica; Pas, Leo; Fitzgerald, Niamh; Ribeiro, CristinaBackground Alcohol is one of the most important risk factors contributing to the global burden of disease. Screening and brief interventions in primary care settings are effective in reducing alcohol consumption. However, implementation of such interventions in routine practice has been proven difficult. Most programmes in practice and research have lacked a theoretical rationale for how they would change practitioner behaviour. Objective To determine whether a theory-based behaviour change intervention delivered to primary care practices significantly increases delivery of alcohol screening. Methods We will conduct a two-arm, cluster-randomized controlled, parallel, open trial. Twelve primary care practices will be randomized to one of two groups: training and support; and waiting-list control. Family physicians, nurses and receptionists will be eligible to participate. The intervention will be a training and support programme. The intervention will be tailored to the barriers and facilitators for implementing alcohol screening and brief interventions following the principles of the Behaviour Change Wheel approach. The primary outcome will be the proportion of patients screened with the Alcohol Use Disorders Identification Test. Conclusion This study will test whether a theory-driven implementation programme increases alcohol screening rates in primary care. Results from this trial will provide a useful addition to existing evidence by informing implementation researchers what areas of behaviour change are critical to increasing alcohol screening rates. Older adults with dementia: knowledge and attitudes of physicians in health units
Publicaçãopor Steiner,Ana Beatriz QuintesOutros Autores: Jacinto,Alessandro Ferrari; Citero,Vanessa de AlbuquerqueOrigem: OasisbrABSTRACT OBJECTIVE: To describe the knowledge and attitudes of general practitioners of the basic health network of the city of São Paulo in relation to patients with dementia and identify patterns of attitudes. METHODS: A total of 10% of the basic health units in the city of São Paulo (n = 45) were randomly distributed into six regional health coordination centers. Up to two general practitioners were interviewed in each unit, with a total of 81 physicians interviewed. They answered the translated and cross-culturally adapted version for Brazil of two British questionnaires, the knowledge quiz (knowledge about dementias) and the attitude quiz (attitude towards the patient afflicted with dementia), as well as a sociodemographic and occupational questionnaire to understand the profile of general practitioners working in primary care. Descriptive data analysis, factor analysis of the main components of the attitude quiz and study of association between attitudes and knowledge were performed, in addition to the multiple linear regression test to determine the relationship between occupational profile and knowledge about attitude patterns in dementia. RESULTS: The physicians interviewed had a median of five-year graduation time; 35.8% worked exclusively with primary care, and less than 40% had completed, or were attending, medical residency or specialization. Physicians showed a lower knowledge about the diagnosis of dementia than about the epidemiology of the disease and its therapeutic management. Their attitudes towards patients afflicted with dementia resulted in four factors: proactive optimism, delegated optimism, implicit dismay, and explicit dismay. The regression study showed that the attitude of explicit dismay decreases the longer the weekly working hours of the physician in the units, and that the delegated optimistic attitude of the physician decreases in the same situation. CONCLUSION: Investment in training is essential to improve physicians' performance in the field of dementia in primary care. -

The Importance of Quinquenary Prevention: A Critical Reflection
Publicaçãopor Loureiro, IsabelOrigem: Gazeta MédicaThe daily life of a family doctor is often about acting and promoting different forms of prevention in patients. In 2014, a new form of prevention emerged - the quinquenary prevention. This prevents harm to the patient, acting on the doctor through strategies that aim to reduce burnout. It is important to reduce burnout because it has consequences for the patient, the physician and, consequently, the healthcare system. In recent years, studies have been conducted regarding the appearance of burnout in physicians compared to other professions. However, it is necessary to point out that this value may be underestimated and the real value may be higher. If the patient needs someone to “look after him”, the doctor needs the same or to be able to take care of himself, since it is assumed that this is the “stable foundation” in the doctor-patient relationship. It is necessary to care for physicians, since happier and more fulfilled professionals will make more appropriate decisions in patient care. The COVID-19 pandemic has increased the prevalence and/or severity of this issue, particularly in primary health care. Thus, the physician needs to take the time to do the quinquenary prevention with realistic goals in mind. There are some burnout interventions that meet this quinquenary prevention and can be applied daily at the physician/healthcare professional, patient, workplace, and administration/ government levels. The self-aware, balanced, energetic and resilient physician is the one who is less susceptible to burnout and the one who contributes to a positive work environment for his/her colleagues.
