Abstract Objective: To describe the outcomes in terms of near visual acuity and quality of life in 10 patients submitted to Scharioth Macular Lens implantation. Setting: João Eugênio Eye Institute, Brasilia, Brazil. Design: Uncontroled clinical trial. Methods: Ten pseudophakic patients with DMRI were submitted to the implantation of intraocular SML in the dominant eye in the period of December of 2017 to the March of 2018. All the patients had answered the Low-Vision Quality of Life Questionaire (LVQOLQ) before and after the surgery, with interval of 30 days. Results: The results points showed a statistical significant improvement (p<0.001) in the near visual acuity (NVA), without interference in the distance vision. All patients had significant improvements (p<0,05) in LVQOLQ on the item that evaluates the near visual acuity. Conclusion: There was an improvement of NVA in all 10 patients, as well as an enhancement to the quality of life in the postoperative period when compared to the preoperative period.

OBJECTIVE: Accurate intraocular lens power calculation may be a challenging issue in eyes with previous corneal refractive surgery. METHODS: The authors present a series of 8 eyes (4 previously submitted to hyperopic laser in situ keratomileusis and 4 to radial keratectomy) submitted to phacoemulsification and implantation of pseudoaccommodating bifocal intraocular lens. Double-K SRK/T formula has been used. RESULTS: Acceptable refractive visual outcomes have been achieved. CONCLUSION: Pseudoaccommodating bifocal intraocular lenses can be safely implanted in patients with previous corneal refractive surgery.

PURPOSE: To determine linear regression models between Alpins descriptive indices and Thibos astigmatic power vectors (APV), assessing the validity and strength of such correlations. METHODS: This case series prospectively assessed 62 eyes of 31 consecutive cataract patients with preoperative corneal astigmatism between 0.75 and 2.50 diopters in both eyes. Patients were randomly assorted among two phacoemulsification groups: one assigned to receive AcrySof®Toric intraocular lens (IOL) in both eyes and another assigned to have AcrySof Natural IOL associated with limbal relaxing incisions, also in both eyes. All patients were reevaluated postoperatively at 6 months, when refractive astigmatism analysis was performed using both Alpins and Thibos methods. The ratio between Thibos postoperative APV and preoperative APV (APVratio) and its linear regression to Alpins percentage of success of astigmatic surgery, percentage of astigmatism corrected and percentage of astigmatism reduction at the intended axis were assessed. RESULTS: Significant negative correlation between the ratio of post- and preoperative Thibos APVratio and Alpins percentage of success (%Success) was found (Spearman's ρ=-0.93); linear regression is given by the following equation: %Success = (-APVratio + 1.00)x100. CONCLUSION: The linear regression we found between APVratio and %Success permits a validated mathematical inference concerning the overall success of astigmatic surgery.

PURPOSE: To evaluate the distance, intermediate and near visual acuity of patients with bilateral cataract operated on both eyes corrected for distance in one eye and for near in the fellow eye, as well as, their stereo acuity and their general satisfaction. SETTINGS: Hospital de Olhos de São Paulo and Complexo Hospitalar Padre Bento - Guarulhos - São Paulo - Brazil. METHODS: This is a prospective study of 76 eyes of 38 patients with bilateral cataract aiming to become spectacle free after surgery. The patients were operated on both eyes; the first eye was corrected for distance and the fellow eye for near vision. The refractive error was programmed between -0.5 D and +0.5 D in the first eye and -2.00 D in the second eye. Patients with preoperative corneal astigmatism equal or higher than 1.0 D were excluded from the study. The uncorrected and best-corrected distant visual acuity (UCDVA, BCDVA) were tested, as well as the uncorrected near visual acuity (UCNVA) and collected at the 3 postoperative months. The Titmus test was performed at the last follow-up. Finally, the modified questionnaire VF-7 was applied and the patient was required to choose, regarding their general satisfaction, between very satisfied, satisfied, or unsatisfied. RESULTS: All patients achieved uncorrected distant visual acuity 20/40 or better (mean SE 0.625 D) and uncorrected near visual acuity J3 or better (mean SE -2.0 D), and intermediate visual acuity J3 in 90%. The Titmus test revealed an average of 197" of arc with reduction of stereo acuity in most of patients as expected and 97.3% of patients demonstrated to be satisfied or very satisfied with this technique. CONCLUSION: It consists in a viable technique for correction of near, intermediate and distant vision on cataract surgery as demonstrated by the high rate of satisfaction (97.3%) by the modified VF-7 questionnaire, especially when the patient has no access to a multifocal intraocular lens.

PURPOSE: To compare the visual outcome, contrast sensitivity and wavefront analysis of patients that underwent cataract surgery and implantation of AcrySof SN60D3 multifocal intraocular lens with those who received the AcrySof SN60AT monofocal IOL. METHODS: This was a prospective clinical trial of forty eyes that received the multifocal IOL and thirty-two eyes that received the monofocal IOL after phacoemulsification. RESULTS: Values for total and spherical aberrations in the multifocal group were statistically lower than in the monofocal group. In the monofocal group, 75% achieved uncorrected intermediate visual acuities between Jaeger 1 and 6. In the multifocal group, 75% of the eyes achieved more than Jaeger 6. At least 87.5% of the multifocal group and 6.3% of the monofocal group achieved monocular uncorrected near acuity of 20/30 (J2, N5) or better. And 90.0% of the eyes in the multifocal group and 37.5% in the monofocal group achieved an uncorrected near acuity of 20/40 (J3, N6) or better. The mean spherical error was 0.11 D in the multifocal group and -0.18 D in the monofocal group (p=0.0379). The SN60D3 group compared to SN60AT group had low contrast sensitivity (log units) with statistically significant differences in 6.0 cpd in photopic conditions (p=0.014) and the SN60D3 group compared to SN60AT group had higher contrast sensitivity (log units) under mesopic conditions (p=0.044). CONCLUSION: The multifocal IOLs induced less spherical aberration than monofocal IOLs and predictably good uncorrected distance and uncorrected near acuities. However, contrast sensitivity was lower in the multifocal group.

Purpose: To compare the effectiveness of limbal relaxing incisions (LRI) with that of toric intraocular lens (IOL) implantation for the treatment of astigmatism during phacoemulsification using nonvectorial (predictability, safety, and efficacy) and vectorial analyses. Methods: This longitudinal observational case series assessed 62 eyes of 31 consecutive cataract patients with preoperative corneal astigmatism of 0.75-2.50 diopters (D) in both eyes. Patients were randomly assigned to 2 groups: a toric IOL group, which received toric IOLs in both eyes, and an LRI group, which received spherical IOLs associated with LRI placement in both eyes. All patients were evaluated at 1, 3, and 6 months after surgery, when refractive astigmatism analysis was performed using both nonvectorial and Alpins vectorial methods. Outcomes were assessed within each group and compared between groups. Results: The proportion of eyes within ±0.50 D of the intended correction at 3 and 6 months after surgery was 75% and 71.88%, respectively, in the LRI group and 40% and 66.67%, respectively, in the toric IOL group. In the remaining period, the proportion was greater in the toric IOL group. The safety index showed no difference at any time point. The efficacy index at 1 and 3 months after surgery was significantly higher in the toric IOL group (0.43 and 0.44, respectively) than in the LRI group (0.31 and 0.36, respectively). At 6 months after surgery, the status of eyes in the LRI group was as follows: 53.13% were undercorrected, 43.74% achieved the intended correction, and 3.13% were overcorrected. In the toric IOL group, 16.76% eyes were undercorrected, 76.67% achieved the intended correction, and 6.67% were overcorrected. The success rates for astigmatic surgery, astigmatism reduction at the intended axis, and astigmatism corrected were 43%, 62%, and 64%, respectively, in the LRI group and 57%, 81%, and 94%, respectively, in the toric IOL group. Conclusions: Our results suggest that the use of toric IOLs may be advantageous over the use of LRIs for the treatment of astigmatism during phacoemulsification. Although such advantages seem subtle in nonvectorial analyses, they are highlighted in vectorial analyses.

Purpose: To assess the postoperative outcomes of a novel toric multifocal in traocular lens (IOL) in patients with cataract and corneal astigmatism. Methods: This prospective nonrandomized study included patients with cataract, corneal astigmatism, and a motivation for spectacle independence. In all patients, a Rayner M-flex® T toric IOL was implanted in the capsular bag. Three months after surgery, the distance, intermediate, and near visual acuities; spherical equivalent; residual refractive astigmatism; defocus curve; and contrast sensitivity were evaluated. A patient satisfaction and visual phenomena questionnaire was administered to all patients. Results: Thirty-four eyes of 18 patients were included in this study. Three months after surgery, the mean corrected distance visual acuity (logMAR) was 0.00 ± 0.08 at 6 m, 0.20 ± 0.09 at 70 cm, and 0.08 ± 0.11 at 40 cm. Uncorrected distance vision acuity was 20/40 or better in 100% eyes. The preoperative mean refractive cylinder (RC) was -2.19 (SD: ± 0.53). After a 3-month follow-up, the average RC was -0.44 D (SD: ± 0.27; p<0.001). Contrast sensitivity levels were high. At the last follow-up, 87.5% patients were spectacle-independent for near, intermediate, and distance vision, and approximately 44% patients reported halos and glare. Conclusion: Toric multifocal IOL implantation in patients with cataract and corneal astigmatism using the Rayner M-flex® T toric IOL was a simple, safe, and accurate option. This technology provides surgeons with a feasible option for meeting patient expectations of an enhanced lifestyle resulting from decreased spectacle dependence.

Purpose: This study was designed to evaluate the visual and anatomical outcomes after cataract surgery in diabetic patients with different intraoperative therapeutic strategies. Methods: The research design comprised of a multicentric, retrospective, interventional study conducted at 6 centers in Argentina, Brazil, Costa Rica, Puerto Rico, Spain, and Venezuela. We included 138 diabetic patients with at least 6-month follow-up following phacoemulsification and intraocular lens implantation. Best-corrected visual acuity (BCVA) and central subfield thickness were collected at baseline and at 1-, 2-, 3-, and 6-month follow-up. Of these, 42 cases were not treated with any intraoperative coadjuvant medication (Group 1), 59 patients received intraoperative bevacizumab (Group 2) and 37 patients received intraoperative triamcinolone (4 mg/0.1 ml) (Group 3). Results: The mean logMAR [± standard deviation (SD)] BCVA improved from 0.82 (± 0.43) at baseline, to 0.14 (± 0.23) at 6-month follow-up (p<0.001) in Group 1; from 0.80 (± 0.48) to 0.54 (± 0.45) (p<0.001) in Group 2; and from 1.0 (± 0.40) to 0.46 (± 0.34) (p<0.001) in Group 3. The mean central subfield thickness increased from 263.57 µm (± 35.7) at baseline to 274.57 µm (± 48.7) at 6-month follow-up (p=0.088) in Group 1; from 316.02 µm (± 100.4) to 339.56 µm (± 145.3) (p=0.184) in Group 2; and from 259.18 µm (± 97.9) to 282.21 µm (± 87.24) (p=0.044) in Group 3. Conclusion: Diabetic patients may significantly benefit from cataract surgery. This study provides evidence to support the use of intravitreal triamcinolone or bevacizumab at the time of cataract surgery in cases with pre-existent diabetic macular edema or moderate-severe non-proliferative diabetic retinopathy.

ABSTRACT Purpose: To analyze the indications for explantation or exchange of intraocular lenses (IOLs), which were originally implanted for the correction of aphakia during cataract extraction. Methods: All cases that involved intraocular lens explantation or exchange in one institution between January 2008 and December 2014 were analyzed retrospectively. Results: In total, 93 eyes of 93 patients were analyzed. The median time interval between implantation and explantation of the anterior chamber intraocular lenses (AC IOL) and posterior chamber intraocular lenses (PC IOL) was 83.40 ± 83.14 months (range: 1-276 months) and 55.14 ± 39.25 months (range: 1-168 months), respectively. Pseudophakic bullous keratopathy (17 eyes, 38.6%) and persistent iritis (12 eyes, 27.8%) in the AC IOL group and dislocation or decentration (30 eyes, 61.2%) and incorrect IOL power (nine eyes, 18.4%) in the PC IOL group were the most common indications for explantation of IOLs. The mean logMAR best corrected visual acuity (BCVA) improved significantly from 1.30 preoperatively to 0.62 postoperatively in the PC IOL group (p<0.001) but did not improve significantly in the AC IOL group (p=0.186). Conclusions: The primary indication for IOL explantation or exchange was pseudophakic bullous keratopathy in the AC IOL group and was dislocation or decentration in the PC IOL group. PC IOL explantation or exchange is safe and improves visual acuity.
