Variables associated with underreporting of AIDS patients, Brazil, 1996
Publicaçãopor Ferreira, Vanja MBOutros Autores: Portela, Margareth C; Vasconcellos, Mauricio TLOrigem: OasisbrOBJECTIVE: The underreporting of AIDS cases in the municipality of Rio de Janeiro, Brazil, is significant. The study intends to analyze the factors associated to this event. METHODS: Using data provided by the Hospital Information System for the year of 1996, in Rio de Janeiro city, and by the National Surveillance System, patients were randomly selected and their medical records reviewed to verify an AIDS diagnosis. A multinomial model was used to perform an analysis of the variations on the chances of underreporting of AIDS cases versus reporting and on the chances of underreporting without evidences to fulfill the case definition of AIDS versus reporting. RESULTS: No significant associations were found between the variables such as "age", "marital status", "level of education", "occupation", and "severity of illness" and the underreporting of AIDS cases. The variable "female gender" showed a strong association with hospitalization without evidence of an AIDS diagnosis. A strong association was found between two or more admissions in an inpatient unit care and reporting. The existence of a epidemiological surveillance department in the hospital is inversely associated with the underreporting of AIDS cases. CONCLUSION: The significant association between organizational variables and underreporting of AIDS cases found in the study point out to the need of standardization of the surveillance procedures, the especial need for the creation and maintenance of surveillance departments in hospitals to improve the quality of the health information system and, therefore, AIDS prevention and care. -

The Brazilian hospital system as a source of information to estimate stillbirth and neonatal mortality rates
Publicaçãopor Schramm, Joyce MAOutros Autores: Szwarcwald, Célia LOrigem: OasisbrOBJECTIVE: Studies on the evolution of infant mortality rate are very relevant. Nevertheless, lack of vital statistics in Brazil limits the temporal and spatial analysis of this indicator. This study aims to investigate the possible use of the Brazilian Hospital Information System as an alternative information source for stillbirth and neonatal mortality rates by age group. METHODS: A new method to estimate the stillbirth and neonatal mortality rates is proposed. It was applied in a set of selected Brazilian states in the year of 1995. For comparative purposes, the Brazilian Death Information System was assessed to estimate the mortality rates under study, after adjusting the registered number of live births by using a demographic tool. RESULTS: By assessing the Hospital Information System a larger number of fetal and early neonatal deaths were observed when compared to data given by the death information system of the Northeastern states. Besides, in the Southern and Southeastern states, where death records are more thorough, the mortality rates calculated using both information sources were very similar. CONCLUSIONS: The results suggest that the proposed methodology could greatly contribute to the analysis of the spatial-time evolution of stillbirth and neonatal death rates in recent years in Brazil, as data on death registration in the majority of the Brazilian states are less thorough than those from the hospital information system. -

The Brazilian Hospital Information System and the acute myocardial infarction hospital care
Publicaçãopor Escosteguy, Claudia CaminhaOutros Autores: Portela, Margareth Crisóstomo; Medronho, Roberto de Andrade; Vasconcellos, Maurício Teixeira Leite deOrigem: OasisbrOBJECTIVE: To analyze the applicability of the Brazilian Unified Health System's national hospital database to evaluate the quality of acute myocardial infarction hospital care. METHODS: It was evaluated 1,936 hospital admission forms having acute myocardial infarction (AMI) as primary diagnosis in the municipal district of Rio de Janeiro, Brazil, in 1997. Data was collected from the national hospital database. A stratified random sampling of 391 medical records was also evaluated. AMI diagnosis agreement followed the literature criteria. Variable accuracy analysis was performed using kappa index agreement. RESULTS: The quality of AMI diagnosis registered in hospital admission forms was satisfactory according to the gold standard of the literature. In general, the accuracy of the variables demographics (sex, age group), process (medical procedures and interventions), and outcome (hospital death) was satisfactory. The accuracy of demographics and outcome variables was higher than the one of process variables. Under registration of secondary diagnosis was high in the forms and it was the main limiting factor. CONCLUSIONS: Given the study findings and the widespread availability of the national hospital database, it is pertinent its use as an instrument in the evaluation of the quality of AMI medical care. -

Quality of data on myocardial infarction deaths, Brazil
Publicaçãopor Melo, Enirtes Caetano PratesOutros Autores: Travassos, Claudia; Carvalho, Marilia SáOrigem: OasisbrOBJECTIVE: To evaluate the quality of data on acute myocardial infarction deaths from mortality information systems. METHODS: Data on in-hospital acute myocardial infarction mortality collected from database of the Mortality Information System (SIM) and Hospital Information System (SIH), in 2000, were analyzed. Then data collected from medical records from two hospitals affiliated to the Unified Health System (SUS) in Rio de Janeiro, Brazil, were also analyzed. Medical records, death certificates, and hospital admission forms (AIH) were compared using the World Health Organization criteria of acute myocardial infarction diagnosis. Agreement among different sources was analyzed using Cohen's Kappa statistics and intraclass correlation coefficient. RESULTS: In-hospital death registries in SIM are much larger than in SIH/SUS. There were identified three mechanisms that could explain most of the observed discrepancy: missing hospital admission forms (32.9%), different main diagnosis registered in SIH/SUS (19.2%), and under reporting of deaths in hospital admission forms (3.3%). The medical records review could confirm the diagnoses of acute myocardial infarction in 67.1% of all deaths reported in death certificates. The sensitivity of data on acute myocardial infarction deaths in death certificates was about 90% for both health information systems analyzed. CONCLUSIONS: There is a need for actions to improve the quality of data registered in SIH/SUS such as standardization of criteria for issuing hospital admission forms during hospital emergencies and training local staff on registration systems. -

Using WeChat to solve the three-long and one-short dilemma: insights from a paradigmatic case
Publicaçãopor Yingfeng, HuangOutros Autores: Alturas, Bráulio; Dias, Gonçalo Paiva; Hong, JiangIn this article, a case-based research, aims to find to what extent social media applications such as WeChat can contribute to solving the dilemma of the three-long and one-short (long waiting time for registration and payment, long waiting time for getting medicine, long waiting time for treatment and short time of diagnosis) dilemma in Chinese large public hospitals. To achieve this objective, the case of implementation of WeChat plus outpatient medical services in the Guangzhou Twelfth Peoples Hospital was studied using a SERVQUAL multiphase inventory measurement. It is concluded that social media-based platforms can effectively contribute to alleviating the three-long part of the dilemma, but solving the one-short will need a more holistic approach. It can be seen that the construction of WeChat + outpatient service platform has improved the patients medical experience, increased the satisfaction of patients and the medical staff, and promoted employees awareness of active service. -

Adverse drug reactions in hospitals: population estimates for Portugal and the ICD-9-CM to ICD-10-CM crosswalk
Publicaçãopor Ascenção, RaquelOutros Autores: Nogueira, Paulo Jorge; Sampaio, Filipa; Henriques, Adriana; Costa, AndreiaBackground: Adverse drug reactions (ADR), both preventable and non-preventable, are frequent and pose a significant burden. This study aimed to produce up-to-date estimates for ADR rates in hospitals, in Portugal, from 2010 to 2018. In addition, it explores possible pitfalls when crosswalking between ICD-9-CM and ICD-10-CM code sets for ADR identification. Methods: The Portuguese Hospital Morbidity Database was used to identify hospital episodes (outpatient or inpatient) with at least one ICD code of ADR. Since the study period spanned from 2010 to 2018, both ICD-9-CM and ICD-10-CM codes based on previously published studies were used to define episodes. This was an exploratory study, and descriptive statistics were used to provide ADR rates and summarise episode features for the full period (2010-2018) as well as for the ICD-9-CM (2010-2016) and ICD -10-CM (2017-2018) eras. Results: Between 2010 and 2018, ADR occurred in 162,985 hospital episodes, corresponding to 1.00% of the total number of episodes during the same period. Higher rates were seen in the oldest age groups. In the same period, the mean annual rate of episodes related to ADR was 174.2/100,000 population. The episode rate (per 100,000 population) was generally higher in males, except in young adults (aged '15-20', '25-30' and '30-35' years), although the overall frequency of ADR in hospital episodes was higher in females. Conclusions: Despite the ICD-10-CM transition, administrative health data in Portugal remain a feasible source for producing up-to-date estimates on ADR in hospitals. There is a need for future research to identify target recipients for preventive interventions and improve medication safety practices in Portugal. -

The impact of perioperative data science in hospital knowledge management
Publicaçãopor Baptista, MárciaOutros Autores: Vasconcelos, José Braga; Rocha, Álvaro; Silva, Rita; Carvalho, João Vidal; Jardim, Helena Gonçalves et al.Conservative practices, such as manual registry have limited scope regarding preoperative, intraoperative and post operative decision making, knowledge discovery, analytical techniques and knowledge integration into patient care. To maximize quality and value, perioperative care is changing through new technological developments. In this context, knowledge management practices will enable future transformation and enhancements in healthcare services. By performing a data science and knowledge management research in the perioperative department at Hospital Dr. Nélio Mendonça between 2013 and 2015, this paper describes its principal results. This study showed perioperative decision-making improvement by integrating data science tools on the perioperative electronic system (PES). Before the PES implementation only 1,2% of the nurses registered the preoperative visit and after 87,6% registered it. Regarding the patient features it was possible to assess anxiety and pain levels. A future conceptual model for perioperative decision support systems grounded on data science should be considered as a knowledge management tool. -

Epidemiological profile of sepsis hospitalizations in Brazil between 2017 and 2021
Publicaçãopor Lins, Anete Nailane SilvaOutros Autores: Olmedo, Letícia Esmério; Ramalho, Ludimila Andrade Gonçalves; Costa, Thais Mendonça da; Castro, Juliana Braga Rodrigues de; Ramos, Ana Paula de SouzaOrigem: OasisbrThe objective was to analyze the epidemiological profile and temporal evolution of hospitalizations, deaths and hospital mortality rate due to sepsis in Brazil between 2017 and 2021. This is an observational, cross-sectional study with a quantitative approach, conducted from the the Hospital Morbidity Information System of the Unified Health System. Data were collected on hospital admissions, deaths and hospital mortality rates for sepsis between 2017 and 2021, according to Brazilian regions, age group, gender, color/race of individuals, and year of care. The results were analyzed using descriptive statistics. A total of 615,805 hospitalizations, 279,765 hospital deaths and a mean hospital mortality rate of 45.49 per 100 hospitalizations per year in Brazil were observed. Both hospitalizations and deaths were more common in the Southeast, among the elderly over 60 years of age, females and caucasians. The mortality rates were higher in the Southeast region, in the age group of 80 years or older, in females and in self-identified blacks. Observing the sex, there was no expressive numerical difference between females and males and regarding color/race, a question mark was raised regarding the completeness of the data. The temporal evolution of hospital admissions and deaths showed an increase until 2019 and reduction until 2021, with hospital mortality rates decreasing until 2019 and increasing until 2021, a result different from those in the literature. Thus, we emphasize the need for further nationwide studies on this grievance, in addition to public policies for early detection of sepsis in this population. -

Characteristics and current direct costs of hospital admissions due to occupational accidents in the southwest of Bahia from 2005 to 2007
Publicaçãopor Silva,Ana Cláudia Conceição daOutros Autores: Pereira,Thalles da Costa LobêOrigem: OasisbrObjective: This study aimed to identify the profile and cost of admissions for occupational accidents, under the Unified Health System (UHS) in municipalities of the southwest of Bahia, in the period of 2005 to 2007. Methods: It was conducted a descriptive study using the records of the Hospital Information System (HIS), from which were extracted data about the sociodemographic, occupational and hospitalization profiles. To express the results, indicators were used as absolute frequencies and proportions, the average stay, Total Cost of Hospitalization (TCH), Hospital Mortality (HM), Average Spenditure (AS) and Cost per Day (CD). Results: 962 admissions were recorded, of which 94.1% were related to path accidents. Among the admitted subjects 65.7% were male, and the most affected age groups were 5 to 14 and 15 to 24 years. There were forearm fractures on 26 cases of typical accidents. 248 cases of intracranial injuries happened during commuting to work, which accounted for 78.2% of traffic accidents and 28.8% of falls. The average stay was of 2.6 days and the deaths occurred in 0.5% of patients discharges. The total cost of the admissions was of R$ 243,125.06, being the AS of R$ 252.73 and CD of R$ 97.44, lower than the external causes. The frequencies of the variables related to the occupation were not verified due to missing values. Conclusion: The data from the HIS Systems were limited for identify the profile of the admitted workers. However, they are important and can be used on occupational health surveillance.
