Introduction: Patients with diabetes have been classified as having a high or very high cardiovascular risk. The 2021 guidelines from the European Society of Cardiology (ESC) emphasize the ongoing need for improved development of risk scores specifically tailored for evaluating cardiovascular risk in individuals with type 2 diabetes mellitus. Acknowledging this necessity, the ESC guidelines of 2023 introduced SCORE2-Diabetes, a predictive model created to assess the 10-year cardiovascular disease (CVD) risk specifically in type 2 diabetes mellitus patients. By combining traditional and diabetes-specific risk factors, this innovative tool aims to enhance risk stratification and management, providing a nuanced approach to prevent cardiovascular diseases in this high-risk population. This study aims to calculate and analyze the changes in cardiovascular risk within a cohort of patients with diabetes, utilizing the SCORE2-Diabetes. Methods: Cross-sectional, observational, descriptive and unicentric study. A cohort of patients aged 18 or older with type 2 diabetes, attending the Day Hospital, was included. Individuals on steroids or erythropoietin-stimulating agents, those undergoing dialysis, pregnant patients, and anyone who could not comply or provide informed consent were excluded. Data was collected to calculate SCORE2-Diabetes, including age, gender, age at diabetes onset, glycated hemoglobin, HDL cholesterol, total cholesterol, LDL cholesterol, creatinine, systolic blood pressure and smoking status. Descriptive and inferential analysis was performed using STATAv18 software. Results: A sample of 60 patients was considered, with 53 ultimately selected for inclusion. The median age was 68 years old (IQR 59; 75), with males constituting 58% of the sample and smokers 11%. Median glycated hemoglobin was 7% (IQR 6.5, 8.1). Utilizing the SCORE2-Diabetes for cardiovascular risk stratification, results showed 8% of patients at low risk, 32% at moderate risk, 44% at high risk, and 16% at very high risk. Notably, 40% of patients were classified as low or moderate risk, a significant finding given that previous guidelines typically categorized individuals with diabetes mellitus as high or very high risk. These downgraded patients were younger, with smaller disease duration and higher estimated glomerularfiltration rate (p <0.05). Conclusion: Our findings reveal a substantial proportion of patients previously categorized as high or very high risk now falling into the low or moderate-risk categories. The simplicity in calculation emphasizes SCORE2-Diabetes potential to refine precision in managing cardiovascular risk for this high-risk population.

Atherosclerotic cardiovascular disease remains the leading cause of morbidity and mortality in developed countries. While the risk factors for atherosclerosis are well established, emerging biomarkers such as lipoprotein(a) [Lp(a)] have gained increasing attention in recent decades. However, its prevalence in the general population remains poorly characterized. This retrospective study analyzed Lp(a) levels in a cohort from a district hospital. A total of 171 measurements were evaluated, with a mean Lp(a) level of 113.38 nmol/L. Notably, 44% of patients exhibited elevated Lp(a) levels, and 7% had markedly elevated values (>400 nmol/L). A higher prevalence was observed among female patients. The population demonstrated a high burden of cardiovascular risk factors, particularly hypertension (79.5%) and diabetes (48.5%). A positive association was found between elevated Lp(a) levels and a history of cardiovascular events. Despite the selected nature of this sample, the findings suggest a higher-than expected prevalence of elevated Lp(a) levels. Given the well-established link between Lp(a) and cardiovascular events, these results highlight the importance of optimizing and intensifying the management of traditional cardiovascular risk factors in patients with elevated Lp(a). It is important to highlight, however, that given the convenience hospital-based nature of the sample, the results cannot be directly generalized to the overall population.

SARS-CoV-2, the novel coronavirus, surprised the world with its capacity of infection, causing a public health emergency concern with more than 3 million people affected in only four months and forcing public institutions to search for ways of obtaining the contention of the virus. The mortality rate is very different among the countries, considering their age structure and the percentage of comorbidities. Elderly and people with underlying diseases are more susceptible to develop severe cases of COVID-19 and have higher case fatality rate. Cardiovascular diseases have a particular importance, given that their prevalence is elevated and considering the infection pathophysiology. Virus uses angiotensin-converting enzyme (ACE) 2 receptors to invade the human cells. These receptors are mainly in the lungs and the heart. Besides the direct viral insult, hypoxia, cytokines storm and catecholamine’s liberation also affect these organs. In the heart, studies have shown that COVID-19 can cause myocarditis, ventricular arrhythmias, acute coronary syndrome and heart failure. Moreover, the cardiovascular insult may be the first manifestation of viral infection in some cases, which is a matter of increased concern during this pandemic. ACE inhibitors and angiotensin receptor blockers (ARB) are drugs of paramount importance in the treatment of cardiovascular diseases. However, studies suggested concern about these medications in COVID-19 because they could cause an ACE2 upregulation and increase the severity of the infection. To the best of our knowledge, no study demonstrated that ACE inhibitors or ARB are harmful and the main cardiovascular societies are recommending the continuity of the treatment.

SARS-COV-2 infection has left no one unscathed or indifferent. It has had profound implications for personal daily lives and health, but also at a broader economic and social level. In terms of cardiovascular disease, COVID-19 had a huge impact. Much remains unknown and the information is often contradictory. This article aims to point out lines of investigation that may be useful to find answers about the link between SARS-COV-2 infection and cardiovascular disease. We explore the known direct effects of the disease, addressing the pathophysiological mechanisms, whether it is the direct cell aggression, inflammatory storm, thrombotic angiopathy, hypoxia or lipoprotein dysfunction.

Introduction: As an effort to reduce mortality associated with acute myocardial infraction (AMI), a fast-track approach named Via Verde Coronária (VVC) was implemented nationwide in 2007. Data on how this approach is performing in different hospitals is still scarce. Our aims were to characterize the main diagnoses of patients included in VVC and to evaluate the proportion of cases diagnosed with AMI. Material and Methods: A retrospective observational study was undertaken in the emergency department of a hospital from Alentejo (06/2017-05/2018). Patients were included if they were aged 18 or older and presented AMI-related symptoms. The main diagnosis was retrieved from medical records and the proportion of AMI was calculated. Data analysis was performed using uni- and bivariate statistics (IBM SPSS v.27.0). Results: A total of 338 patients were included, where 56.5% (n = 191) were male with a mean age of 63.9 ± 15.1 {19;96} years old. The majority of diagnoses were from the circulatory system (61.2%, n = 207). AMI was present in 27.2% (n = 92/338) of the sample. There were differences when comparing the number of risk factors between patients with AMI and non-AMI (2.83 ± 1.54 vs 2.06 ± 1.49 risk factors/patient, respectively; p < 0.001) and in patients with NSTEMI and STEMI (3.25 ± 1.36 vs 1.81 ± 1.50 risk factors/patient, respectively; p < 0.001). There was a statistically significant difference when analyzing the duration of pain between patients with AMI and non-AMI (6.24 ± 9.08 vs 10.53 ± 18.23 hours of pain, respectively; p = 0.032). Conclusion: Data suggest that the majority of diagnosis in VVC was related to the circulatory system, and a quarter of them were AMI.

Introduction: Cardiovascular disease is a leading cause of death and chronic disability and optimizing its prevention and management remains a priority. This review aims to provide a critical analysis on the positive and negative aspects concerning the impact of a vegetarian diet in cardiovascular risk. Results: Overall, vegetarian population presents better cardiovascular risk profile, expressed by lower body mass index, better blood pressure control, reduced pro-atherogenic lipids and better glycaemic control and insulin sensitivity. Other atherogenic paths seem to be influenced, such as chylomicron remnants removal from circulation, oxidative and inflammation profile, blood fluidity and intestinal microbiota. Furthermore, imaging methods have shown better structural and functional vas- cular properties among vegetarians. However, a non-balanced vegetarian diet might lead to nutrients deficit, which could nullify these advantages. These limitations may be overcome through a carefully planned diet and, in some cases, supplements or fortified foods. Ultimately, a beneficial effect is associated with a healthy vegetarian concept, with scarce intake of refined and processed food products, avoiding overconsumption of sugar and trans fats. Conclusion: Vegetarian diet brings a positive impact in sever- al independent cardiovascular risk factors. Despite the additional challenge in reaching specific macro and micronutrients which are less available in plant-based foods, their shortage can be avoided by planning a well-balanced and complete diet, based on healthy and natural food components. As there is the acknowledgment of its limitations and corresponding precautions are taken, a vegetarian diet could be used as an effective weapon towards prevention and management of cardiovascular disease.

Seguindo as guidelines PRISMA triámos trinta e nove artigos científicos, selecionando no final sete.
