Introduction: Hyperkalemia (HK) is characterized by elevated serum potassium levels and is common in patientswith chronic kidney disease, diabetes mellitus and cardiovascular diseases. Severe hyperkalemia increases the risk of cardiac disturbances and mortality. Real-world studies and international expert groups highlight the urgent need to optimize hyperkalemia management however, consensus in this area is lacking, especially concerning new drugs. We aim to establish a consensus among healthcare professionals in Portugal regarding the effective management of HK in emergency settings. Additionally, we aim to provide recommendations for the adoption of best practices in emergency care. Methods: This qualitative study was based on Delphiconsensus. A panel of experts collaboratively identified anddiscussed five main topics related to HK management anddefined 25 treatment statements. An online questionnaire was distributed to doctors who frequently handle emergencies between July and August 2023. A consensus was defined when at least 65% of respondents indicated agreement or strong agreement with a statement. The initial round produced high agreement levels, obviating the need for additional survey rounds. Results: A total of 63 specialist doctors participated in the online questionnaire. There was a very high consensus, with over 90% of participants either agreeing or stronglyagreeing with 19 of the 25 statements. These establish thatHK is a critical concern for patients with chronic kidney disease, particularly those with a lower estimated glomerular filtration rate (eGFR), elderly population and patients on medications such as inhibitor renin-angiotensin–aldosterone system (iRAAS), angiotensin II receptor blockers ( ARBs), mineralocorticoid-receptor antagonists (MRAs), or potassium-sparing diuretics. They also agree that HK is associated with higher mortality risk, which increases in individuals with underlying conditions or prolonged HK. Statements regarding potassium management involving sodium zirconium cyclosilicate (SZC), patiromer, scavengers in patients on iRAAS, and related to insulin treatment had the lowest consensus levels. Conclusion: A robust consensus among Portuguesehealthcare professionals regarding effective HK management in emergency settings was achieved. Experts collaboratively formulated recommendations covering key aspects of the management of HK in emergency settings. Implementing these recommendations will enhance the delivery of harmonized evidence-based medical care and the improvement of clinical outcomes.

Aim: To develop consensus recommendations about good clinical practice rules for caring end-of-life patients. Methods: A steering committee of 12 Spanish and Portuguese experts proposed 37 recommendations. A two rounds Delphi method was performed, with participation of 105 panellists including internists, other clinicians, nurses, patients, lawyers, bioethicists, health managers, politicians, and journalists. A questionnaire with 5 Likert-type answers for each recommendation was sent to panellists. Strong consensus was defined as > 95% answers completely agree or > 90% agree or completely agree; and weak consensus when > 90% answers completely agree or > 80% agree or completely agree. Results: The panel addressed seven specific areas for 37 recommendations spanning: Identification of patients; Knowledge of the disease, Patient’s Values and preferences; Information; Patient’s needs; Support and care; Palliative sedation, and After death care. Conclusions: The panel formulated and provided the rationale for recommendations on good clinical practice rules for caring end-of-life patients.

Myelodysplastic syndromes (MDS) are a diverse group ofmyeloid neoplasias, characterized by cytopenias stemmingfrom ineffective hematopoiesis and by an increased risk ofprogression to acute myeloid leukemia. They affect 5/100000 people per year in Portugal, among which more than60% will become transfusion-dependent during the courseof the disease. Cytopenias may lead to significant morbidityand their correct management is the key to maintain thesepatients’ quality of life. This document gathers the result ofthe debate promoted by the Anemia Working Group Portugalwith experts on the management of patients with lower riskMDS, addressing the medium- and long-term effects of recurrent transfusion and iron overload. For the diagnosis of MDS,the group recommends the use of World Health Organizationclassification published in 2016. The International PrognosisScoring System score is still the reference score to defineboth the prognosis and the therapeutic strategy. Supporttherapy is a fundamental cornerstone in the managementof MDS patients at any stage of the disease, and includesantibiotics, transfusion of erythrocytes and platelets, hematopoietic growth factors, antifibrinolytic agents and chelators.Chelation therapy should be considered in patients transfused with ≥ 20 units of erythrocytes or with serum ferritin ≥1000 ng/mL. Patients who are candidates to chelation shouldbe submitted to periodic magnetic resonance imaging to theliver and the pancreas. Therapeutic options should take intoaccount the individual characteristics of the disease for everypatient and preserve the quality of live, in accordance withinternational guidelines.

A OSCE entre a (Falta de) Personalidade Jurídica e o Consenso como Forma de Legitimidade Política

    Publication
    by Soares, Beatriz Godinho
    Origin: uBibliorum

    Publication
    by Elias, Tânia de Sousa
    Origin: Oasisbr

    Publication
by Aguiar, Celso Azoury Telles de
    Origin: Oasisbr
