Nurses' experiences in caring for children at the end of life and their families: a descriptive-exploratory study
Publicaçãopor Real , AnaOutros Autores: Cerqueira , Manuela; Sousa, TeófiloOrigem: Onco.NewsDespite the technological advances in the diagnosis and treatment of paediatric diseases, death in childhood remains a persistent reality. National evidence portrayed in the paediatric services of our hospitals strengthens the urgency to create Paediatric Palliative Care. The aim of this study is to understand the implications to nurses working in a Paediatric Unit of caring for children in the end of life and their families. Methodological strategies consisted in following a qualitative approach with a descriptive-exploratory type of study, using semi-structured interviews as a tool for data collection and treat data through content analysis. The sample is made up by 11 nurses practicing in Oporto’s Portuguese Institute of Oncology Paediatric Service. The results obtained show that the caring experience towards children in the end of life and their families arises a broad connection of feelings/emotions and peculiar thoughts, intrinsic to the therapeutic relationship which is significantly marked by the awareness of professional responsibility, gratification of good memories and non-acceptance of the death. Express the meaning of pediatric palliative care, as symptom control, quality of life and, particularly, as non-curative care. The end-of-life meaning described focuses on organic/physical death and uncontrolled disease. -

Promotion of child development in children/adolescents with oncological disease: integrative review of the literature
Publicaçãopor Rodrigues Pita, Noélia MariaOutros Autores: Marques, Goreti; Silva, SofiaOrigem: Onco.NewsIntroduction: The diagnosis of oncological disease in pediatric age imposes challenges on the child/adolescent and family, due to the disease and the entire treatment process, limiting their normal development in different dimensions: physical, motor, cognitive, psychological, emotional, and social. Objective: To analyze the scientific evidence regarding interventions by Nurse to promote child development in children/adolescents with oncological disease.Methodology: Integrative Literature Review; data collection on June 1, 2023, in the MEDLINE® Complete, CINAHL® Complete, Nursing & Allied Health Collection: Comprehensive and Cochrane Central Register of Controlled Trials. Inclusion criteria: articles produced in the last 10 years, in Portuguese, English and Spanish. Results and Discussion: The final sample included five articles, in which it appears that nurses recognize the need to include the assessment and promotion of child development as an integral part of care, in the domains that comprise it: physical, cognitive, social, psychological, and emotional. Conclusion: The awareness of the Nurse in promoting child development in children with oncological disease is fundamental, as it allows them to engage in a humanized way with the child/young person and their family, contributing to adaptation to the treatment/disease process, reducing their impact on the life of the child/adolescent and family. -

Cluster Stability as a New Method to Assess Changes in Performance and its Determinant Factors Over a Season in Young Swimmers
Publicaçãopor Morais, JorgeOutros Autores: Silva, António; Marinho, Daniel; Seifert, Ludovic; Barbosa, Tiago M.Origem: uBibliorumTo apply a new method to identify, classify, and follow up young swimmers based on their performance and its determinant factors over a season and analyze the swimmers' stability over a competitive season with that method. Fifteen boys and 18 girls (11.8±0.7 y) part of a national talent-identification scheme were evaluated at 3 different moments of a competitive season. Performance (ie, official 100-m freestyle race time), arm span, chest perimeter, stroke length, swimming velocity, speed fluctuation, coefficient of active drag, propelling efficiency, and stroke index were selected as variables. Hierarchical and k-means cluster analysis were computed. Data suggested a 3-cluster solution, splitting the swimmers according to their performance in all 3 moments. Cluster 1 was related to better performances (talented swimmers), cluster 2 to poor performances (nonproficient swimmers), and cluster 3 to average performance (proficient swimmers) in all moments. Stepwise discriminant analysis revealed that 100%, 94%, and 85% of original groups were correctly classified for the 1st, 2nd, and 3rd evaluation moments, respectively (0.11≤Λ≤0.80; 5.64≤χ2≤63.40; 0.001<P≤.68). Membership of clusters was moderately stable over the season (stability range 46.1-75% for the 2 clusters with most subjects). Cluster stability is a feasible, comprehensive, and informative method to gain insight into changes in performance and its determinant factors in young swimmers. Talented swimmers were characterized by anthropometrics and kinematic features. -

Effects of the pitch configuration design on players’ physical performance and movement behaviour during soccer small-sided games
Publicaçãopor Coutinho, DiogoOutros Autores: Gonçalves, Bruno; Santos, Sara; Travassos, Bruno; Wong, Del P.; Sampaio, JaimeOrigem: uBibliorumThis study aimed to identify the effects of different pitch configurations on youth players positional and physical performances. Forty players participated in a Gk + 5vs5 + Gk small-sided game under four conditions: regular condition (regular), pitch with the direction of competitive matches; sided condition (sided), goals were changed to width; different pitch orientation (≠orientation), performed in side-to-side line compared to competitive matches; dynamic pitch (dynamic), boundaries were randomly changed every minute by: regular pitch; decrease 6 m width; diamond shape. The following variables were considered: players' effective playing space, distance between teammates' dyads time spent synchronized, average speed and a ratio between the distance covered at different intensities and distance covered while recovering. Overall, players exhibited better performances in pitches that are more representative of the environmental information seen during competitive matches (regular and ≠orientation). However, coaches may also use different boundary conditions to promote the players' ability to adapt to different context information. -

Modeling the Links Between Young Swimmers’ Performance: Energetic and Biomechanic Profiles
Publicaçãopor Barbosa, Tiago M.Outros Autores: Costa, Mário Jorge; Marinho, Daniel; Coelho, Joel; Moreira, Marc; Silva, AntónioOrigem: uBibliorumThe aim was to develop a path-flow analysis model for young swimmers' performance based on biomechanical and energetic parameters, using structural equation modeling. Thirty-eight male young swimmers served as subjects. Performance was assessed by the 200-m freestyle event. For biomechanical assessment the stroke length, the stroke frequency and the swimming velocity were analyzed. Energetics assessment included the critical velocity, the stroke index and the propulsive efficiency. The confirmatory model explained 79% of swimming performance after deleting the stroke index-performance path, which was nonsignificant (SRMR = 0.06). As a conclusion, the model is appropriate to explain performance in young swimmers. -

Secular Trends in Anthropometrics and Physical Fitness of Young Portuguese School-Aged Children
Publicaçãopor Costa, Aldo M.Outros Autores: Costa, Mario; Reis, António Antunes; Ferreira, Sandra S.; Martins, Júlio; Pereira, AnaOrigem: uBibliorumIntroduction: The purpose of this study was to analyze secular trends in anthropometrics and physical fitness of Portuguese children. Material and Methods: A group of 1819 students (881 boys and 938 girls) between 10 and 11 years old was assessed in their 5th and 6th scholar grade throughout a 20 years’ time-frame. ANCOVA models were used to analyze variations in anthropometrics (height, weight and body mass index) and physical fitness (sit and reach, curl-up, horizontal jump and sprint time) across four quinquennials (1993 - 1998; 1998 - 2003; 2003 - 2008; 2008 - 2013). Results: Secular trends showed the presence of heavier boys and girls with higher body mass index in the 5th and 6th grade throughout the last 20 years. There was also a presence of taller girls but just until the 3rd quinquennial. Both boys and girls were able to perform better on the core strength test and sprint time but become less flexible over the years. Mean jumping performance remained unchanged for both genders. Discussion: The present study provides novel data on anthropometrics and physical fitness trends over the last two decades in young Portuguese children, consistent with the results reported in other developed countries. Conclusion: Evidence for the start of a positive secular trend in body mass index and in some physical fitness components over the last two decades among the Portuguese youth. -

The relative age effect among elite youth competitive swimmers
Publicaçãopor Costa, Aldo M.Outros Autores: Marques, Mário C; Louro, Hugo; Ferreira, Sandra S.; Marinho, DanielOrigem: uBibliorumThe aim of this study was to analyse the relative age effect (RAE) in competitive swimming. The best 50 Portuguese swimmers (12- to 18-year-olds) for the main individual swimming pool events of both genders were considered. Analysis was conducted on 7813 swimming event participants, taking account of respective swimmer birth dates and the Fédération Internationale de Natation points gained. Differences in the distribution of birth dates by quarter year were determined using the Chi-square. A one-way analysis of variance ANOVA was used to test for differences measured in points between individuals by quarterly birth year intervals. A two-way analysis of variance ANOVA was also conducted to test the interaction between gender and seasonal birth date with regard to performance. The results show an inequitable distribution (p<0.01) of birth dates by quarter for almost all age groups and both genders. However, the distribution of birth dates by quarter for each considered swim event shows that RAE seems to exist only for 12-year-old females and 12- to 15-year-old males. Analysing mean swimming performance, post-hoc results (p<0.01) show no consistency in RAE. Higher performance occurs among older swimmers only in 100 m butterfly (female 1998, 1st≠2nd quarter, p=0.003). The results also show no interaction between gender and seasonal birth date (p<0.01). Findings of this study show that a higher number of swimmers, particular males, are born in the first two quarters of the year, although there is mostly no effect of seasonal birth date on performance differences within the top 50 swimmers. -

Growth influences biomechanical profile of talented swimmers during the summer break
Publicaçãopor Moreira, Marc F.Outros Autores: Morais, Jorge; Marinho, Daniel; Silva, António; Barbosa, Tiago M.; Costa, Mário JorgeOrigem: uBibliorumThis study aimed to analyse the effect of growth during a summer break on biomechanical profile of talented swimmers. Twenty-five young swimmers (12 boys and 13 girls) undertook several anthropometric and biomechanical tests at the end of the 2011-2012 season (pre-test) and 10 weeks later at the beginning of the 2012-2013 season (post-test). Height, arm span, hand surface area, and foot surface area were collected as anthropometric parameters, while stroke frequency, stroke length, stroke index, propelling efficiency, active drag, and active drag coefficient were considered as biomechanical variables. The mean swimming velocity during an all-out 25 m front crawl effort was used as the performance outcome. After the 10-week break, the swimmers were taller with an increased arm span, hand, and foot areas. Increases in stroke length, stroke index, propelling efficiency, and performance were also observed. Conversely, the stroke frequency, active drag, and drag coefficient remained unchanged. When controlling the effect of growth, no significant variation was determined on the biomechanical variables. The performance presented high associations with biomechanical and anthropometric parameters at pre-test and post-test, respectively. The results show that young talented swimmers still present biomechanical improvements after a 10-week break, which are mainly explained by their normal growth. -

Parents’ educational level and second-hand tobacco smoke exposure at home in a sample of Portuguese children
Publicaçãopor Vitória, PauloOutros Autores: Nunes, Célia; Precioso, J.Origem: uBibliorumSecond-hand tobacco smoke (SHS) exposure is a major and entirely avoidable health risk for children's health, well-being and development. The main objective of the current study was to investigate the association between parents' educational level and children's SHS home exposure. A self-administered questionnaire was conducted within a sample of 949 students in 4th grade (mean age 9.56±0.75, 53.4% male). The sample was randomly selected from all schools located at Lisbon District, Portugal. The current study confirmed that Portuguese children are exposed to unacceptable high levels of SHS at home, mainly by their parents' smoke. Prevalence of smokers was higher amongst parents with low educational level. Children of parents with low educational level were more likely to suffer SHS exposure at home. These results confirmed the social inequalities associated with smoking, support the relevance of more research on this subject and stress the need for more interventions to control this problem. Some interventions should be specifically aimed at less educated parents, particularly at less educated mothers. -

Physical Fitness Differences Between Prepubescent Boys and Girls
Publicaçãopor Marta, Carlos C.Outros Autores: Marinho, Daniel; Barbosa, Tiago M.; Izquierdo, Mikel; Marques, MCOrigem: uBibliorumThe purpose of this study was to analyze in which physical capabilities boys and girls are closer or distant. An additional objective was to find which of the body fat, physical activity, and somatotype factors have a greater effect on prepubescent children's physical fitness. This was a cross-sectional study involving 312 children (10.8 ± 0.4 years). The physical fitness assessment employed sets of aerobic fitness, strength, flexibility, speed, agility, and balance. The boys presented higher values in all selected tests, except tests of balance and flexibility, in which girls scored better. Gender differences in the physical fitness were greatest in the explosive strength of upper (p ≤ 0.01, η(p)(2) = 0.09) and lower limbs (p ≤ 0.01, η(p)(2) = 0.08), although with a medium-size effect of gender, and smaller in the abdominal (p > 0.05, η(p)(2) = 0.007) and upper limbs (p > 0.05, η(p)(2) = 0.003) muscular endurance, and trunk extensor strength and flexibility (p > 0.05, η(p)(2) = 0.001). The endomorphic (p ≤ 0.01, η(p)(2) = 0.26) in the girls, and the ectomorphic (p ≤ 0.01, η(p)(2) = 0.31) and mesomorphic (p ≤ 0.01, η(p)(2) = 0.26) in the boys, had the high-sized effect on the physical fitness. The physical activity in the girls, and the endomorphic and body fat in the boys, did not have a significant effect. These findings can help in the planning of activities that take into account the success and motivation of both boys and girls and thus increase levels of physical activity and physical fitness at school. However, in prepubescent children, one cannot neglect the influence of genetic determinism, observed from the morphoconstitutional point of view.
