Background: Treatment with stem cells in various cardiomyopathies may be related to the increase in arrhythmias. Objective: To determine whether intracoronary injection of stem cells in patients with Chagas cardiomyopathy is associated with increased frequency of ventricular arrhythmias, compared to the control group. Methods: A retrospective cohort study that evaluatedt he medical records of 60 patients Who participated previously on sectional study. The following data was collected : age, sex , and drugs used and Holter variables that demonstrated the presence of arrhythmias.Holter was performed in four stages: randomization , 2 , 6 and 12 months segments. The control group (CG) received medical treatment and intracoronary injection of placebo and the study group (SG) drug treatment and autologous stem cell implant. Results: There was no difference between EG and CG analyzing the criteria of arrhythmia. In the intra-group analysis, significant difference was found between the Holter tests study group in the variable of total ventricular ectopic compared with baseline, being between H1 and H2 p=0.014, between H1 and H3 p=0.004, between H1and H4 p=0.014. The variable non-sustained ventricular tachycardia between H1 and H3 with p = 0.036. Conclusion: The intracoronary injection of stem cells did not increase the incidence of ventricular arrhythmias in patients with Chagas cardiomopatia compared to the control group.

    Publication
by Lima, Daniel José Matos de Medeiros
    Origin: Oasisbr

Development of functional food targeting cardiometabolic risk

    Publication
    by Silva, Andreia Filipa Rodrigues
Diet is the most efficient non -pharmacological strategy for preventing cardiometabolic risk, the main factor in the high incidence of cardiovascular diseases, namely diabetes. Hydroxytyrosol, oleuropein , and resveratrol have bioactive properties to prevent this risk. This work aimed to promote the introduction of these compounds into food as part of new functional foods. Hydroxytyrosol and oleuropein have already been explored in foods as natural preservative s. To meet their demand and promote alternative sources, approaches were exploited for recovering these compounds from olive leaves. It was possible to recover hydroxytyrosol and mostly oleuropein through extraction with acidified water at room temperature, becom e more stable after the purification step, and selectively recover hydroxytyrosol by subjecting the aqueous extract to the acid hydrolysis step. On the other hand, until now, the application of resveratrol has been scarce and presents difficulties due to its instability and hydrophobicity. Several delivery vehicles have been used in an attempt to protect and increase the bioaccessibility of resveratrol. However, it is still not clear whether they remain efficient when introduced into food matrices. In this work, resveratrol was complexed with γ -cyclodextrin (γ -CD·RSV·14H₂O), and with β -cyclodextrin (β -CD·RSV·9.5H₂O) aiming to increase its solubility. γ - CD·RSV·14H₂O promoted the solubility and stability of resveratrol in lemon juice over 28 days. Moreover, the use of both complexes in rice beverages allowed for an increase in the resveratrol bioaccessible fraction under digestion conditions. On a second approach to this work, it was studied the interaction of starch with resveratrol which, when applied at 2%, was fully integrated into the starch matrix without significantly interfering with its digestibility and carbohydrates digestive enzyme activity, thus ensuring its introduction into bread. Wheat bread was formulated with 0.5% resveratrol in its free form, complexed with γ -cyclodextrin and encapsulated in zein nanoparticles. Although both vehicles helped to retain resveratrol in the bread dough and preserve its antioxidant activity, they did not increase its final bioaccessibility or bioavailability. This suggest s that the bread itself acts as a vehicle. The preventive effect of this bread was evaluated for the prevention of diabetic cardiomyopathy in Sprague Dawley rats by adding it to the animals’ diet at a dosage of 10 mg resveratrol/kg body weight. The results showed that a diet with resveratrol bread decreased polydipsia and body weight loss in the early stages of type 2 diabetes and fibrosis in the heart. Thus, this part of the work allowed the development of a functional bread by incorporating resveratrol , which proved to be an effective vehicle not only to introduce resveratrol into food but also as a dietary intervention to control the early stages of type 2 diabetes and its associated complications.

Other Authors: Zaidel, Ezequiel; Acquatella, Harry; Barbosa, Marcia Melo; Narula, Jagat; Nakagama, Yu et al.
Chagas disease (ChD) represents a significant health burden in endemic regions of Latin America and is increasingly being recognized as a global health issue. The cardiac involvement in ChD, known as Chagas cardiomyopathy (ChCM), is the most severe manifestation and a leading cause of heart failure and mortality in affected individuals. Echocardiography, a non-invasive imaging modality, plays a crucial role in the diagnosis, monitoring, and risk stratification of ChCM. This consensus recommendation aims to provide guidance on the appropriate use of echocardiography in ChD. An international panel of experts, including cardiologists, infectious disease specialists, and echocardiography specialists, convened to review the available evidence and provide practical recommendations based on their collective expertise. The consensus addresses key aspects related to echocardiography in ChD, including its role in the initial evaluation, serial monitoring, and risk assessment of patients. It emphasizes the importance of standardized echocardiographic protocols, including the assessment of left ventricular function, chamber dimensions, wall motion abnormalities, valvular involvement, and the presence of ventricular aneurysm. Additionally, the consensus discusses the utility of advanced echocardiographic techniques, such as strain imaging and 3D echocardiography, in assessing myocardial mechanics and ventricular remodeling.

Introduction and Aims: The functional significance of coronary lesions can be assessed in the cardiac catheterization laboratory by determination of fractional flow reserve (FFR), thus overcoming one of the major limitations of conventional angiography. The aim of this study was to analyze the long-term clinical course of patients with intermediate coronary stenosis (50---70%) deferred for intervention based on FFR <0.80. Methods: Between May 1999 and December 2009, 300 lesions in 231 patients (mean age 65±10 years, 68% male and 75.3% with multivessel disease) were studied by FFR. Intervention was deferred in 282 (94%) lesions and 18 were treated based on FFR <0.80. We assessed the occurrence of major adverse cardiovascular events (MACE), defined as cardiovascular death, non-fatal acute coronary syndrome and target lesion revascularization (TLR). Results: During a median follow-up of 637 days (interquartile range 455---1160), there were 15 (6.5%) MACE in the subgroup of patients with target lesion intervention deferred based on FFR: one cardiovascular death, four hospitalizations for acute coronary syndrome and 14 TLR (12 patients were treated percutaneously and two underwent coronary artery bypass grafting). MACE-free survival at one year follow-up was 97.8%. Conclusion: These results, in a real-world population, support the current trend to base the decision to treat on functional rather than purely anatomical criteria, in order to improve safety and efficiency.

    Publication
by Gonçalves, Mafalda Portugal Ribeiro Pires

    Publication
    by Santo, Sofia Brandão
Contudo, é difícil predizer a doença antes do tratamento.
